8 The handwash sink in the second floor food prep area was blocked by a container/bucket and not accessible at all times for employee use. Corrective action: Container relocated.
14 -The can opener had an accumulation of food residue on the food contact surfaces. -Three-bay sink in the second floor food prep area, used to wash produce, observed soiled. Wash, rinse, sanitize and air-dry the equipment.
26 One package of loose rodent bait observed on the floor in the basement. Corrective action: Package discarded. ***Note: Rodent bait must be contained in covered, tamper-resistant bait stations.
36 Visible physical evidence of rodent activity observed in the facility. 1. Mouse feces was observed in the following areas: -Floors perimeter in the second floor food prep area. -Cabinet in first floor prep area near front window. -Cabinet beneath the handwashing sink in the first floor toilet room. -Floor in the second floor toilet room. -Floor in grime near inoperable walk-in on second floor. -Basement floor. -Floor in closet in rear. 2. Dead mice in gap in step near door leading to the rear of the establishment.

45 -Multiple pieces of unused/inoperable equipment were observed in the facility. (i.e. walk-in cooler, stand mixer, reach-in refrigerators, etc.) -Condensation line observed in second basin of 2-bay sink with sanitizer solution. Per owner, sanitizer is used for wiping cloths. *Note: If second basin is used for condensation line, it cannot be used for sanitizer solution. Prepare sanitizer solution in a clean container instead. -Duct tape observed on chest freezer door. -Garbage grinder is attached to a wall-mounted, shallow-basin sink rather than a free-standing, NSF approved, deep-basin sink.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Cleaning: -Black residue observed on the interior back and side walls of the reach-in refrigerator in the point of sale area. -Debris and rust observed on racks in walk-in cooler. -Remove mouse droppings from cabinets.

14 -Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. -The can opener had an accumulation of food residue on the food contact surfaces. -Mouse feces observed on shelves beneath tables where equipment and containers of spices were observed stored. -Mouse feces observed on stool on top of cutting board in the food prep area. Wash, rinse, sanitize and air-dry equipment (slicer, can opener, cutting boards), exterior of spice containers, and shelves beneath tables. ***Note: Ensure that spices are not contaminated by water or cleaners during cleaning.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 150 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.

Rodent bait was observed on the counter directly beneath the slicer. -Loose rodent bait observed on the floors throughout the establishment. ***Note: Rodent bait must be contained in covered, tamper-resistant bait stations.
