Note: The person in charge was instructed to submit a plan review application for the solid fuel-charcoal grill. The grill was not in use.

8 Soap was not observed at the hand wash station in the bar area.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Food safety certificated person was not present at Sanitarian's arrival. Food safety certificated person arrived during the inspection.
8 No sign or poster posted at the handwash sink in the basement bar area to remind food employees to wash their hands.
12 Shellstock located in the walk-in cooler area did not have identification tags attached to the container.
**Shellstock tags shall remain attached to the container in which the shellstock are received until the container is empty.
14 - Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
- Black residue observed on the interior of the ice machine bin in the bar area.
26 Loose rodenticides packages observed on the floor and not contained in a covered, tamper-resistant bait station in the alcohol storage room.
26 Working containers in bar area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. PIC labeled the bottle during the inspection.
33 Food being thawed using an unapproved method. Food observed thawing in standing water.

8 Soap dispensers were empty at the handwash sinks in the basement bar, prep area, and men's restroom (basement). Soap was refilled.
Handwash reminder sign was missing from the basement bar area. Sign was provided.
13 Observations:
- Raw animal food (fish) was stored above ready to eat foods in the walk-in cooler.
- Raw chicken was stored above raw beef. Foods were rearranged.
45 Foil observed lining shelves/surfaces in the prep area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 General cleaning needed in the walk-in freezer due to food residue and debris observed.
51 Dust accumulation observed on the vent cover in the men's restroom in the main bar area.
