18 Chicken, was not cooled from 135 degrees F to 70 degrees F in 2 hours and/or from 135 degrees F to 41 degrees F within 6 hours, after preparation. Food was relocated.

20 Potentially hazardous food(chicken) was held at 46 degrees F rather than 41 degrees F or below as required. Chicken was relocated to the walk-in box.

37 Beverages were not stored at least 6 inches above the floor.

45 Observations: - Soda crates being used for elevation in the waiting area and walk-in box - Cardboard lining shelf in prep area

45 - An Non NSF approved chest freezer was observed in the food preparation area.

47 The following observations were made: - Food debris on bottom surface of reach-in freezer in kitchen area. - Dust accumulation on fan vent covers in walk-in cooler. - Food debris on flooring surface of walk-in boxes. - Polyfilm observed lining a refrigerator in the prep area.

49 A mop sink was not observed.

51 General cleaning needed: - Debris on the customer restroom floor - Dust accumulation on the vent cover

53 - Damaged floor tiles observed throughout the facility.

53 Broom, dustpans, and mops were not elevated on a utility rack when not in use.

53 Stagnant water observed in a mop bucket.

54 Hood filters have an accumulation of static dust and grease.
