21 Lasagna, deli meats and cheeses, cut lettuce and cut tomatoes, potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler and cold preparation units was not date marked. Corrective action was taken and date items were date marked.
26 Working containers in back preparation area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical. Person in Charge took corrective action and labeled bottle.
41 Serving utensils located in ice machine, stored with food contact surfaces exposed to hand contact when retrieved for service. Person in Charge took corrective action and removed ice scoop from bin.

8 Soap and paper towels was not available at the handwash sink in the 2nd floor preparation/ warewash area. Person in Charge took corrective action and removed items.
13 Raw eggs were stored above ready to eat foods in the walk-in cooler. Person in Charge took corrective action and relocated items.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher (2nd floor and 1st floor dishwasher) was 0 ppm, and not 50-100 ppm as required. Dishwashers were repaired during time of inspection.
20 Risotto balls, stuffed eggplant, baked ziti and sauce was held between 45-52 degrees F rather than 41 degrees F or below as required. Risotto balls and ziti were discarded and other food items were relocated.
36 Open bait located in 2nd floor preparation area and 2nd floor storage/utility room with potential to contaminate food, equipment, and / or utensils. Bait must be stored in an enclosed bait box.
36 Visible physical evidence of rodent activity observed in the 2nd floor preparation/ warewash area.

8 -No sign or poster posted at the handwash sink in one of two restrooms to remind food employees to wash their hands. -Soap was not available at the handwash sink in the front gelato scooping area. Person in Charge provided soap.
13 Uncovered bar ice observed at bar.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwashers at the bar and warrewash area was 0 ppm, and not 50-100 ppm as required. Person in Charge took corrective action and machines were repaired during time of inspection.
14 Wiping cloths observed stored in sanitizer bucket with sanitizer concentration of 500 ppm rather than 200 ppm. Sanitarian advised Person in Charge to adjust chemical concentrations.
20 Cut tomatoes, soup, baked ziti, shrimp, chicken, pasta, pork, and deli meats was held between 46-58 degrees F rather than 41 degrees F or below as required. Shrimp was relocated to another refrigerator. Sanitarian advised Person in Charge to discontinue using cold hold units until fixed. Person in Charge took corrective action and turned down walk-in cooler. See discard list.

47 -Debris observed on interior surfaces of storage cabinets in front food preparation area. -Residue and debris observed on storage shelving under preparation tables, dishwashers, and inside surfaces of refrigerators. -Wire storage shelving area not clean on sight. -Food residue observed under handles of refrigerators.
51 Hand wash sink is not fixed and sealed to wall.
53 -Debris and mice droppings observed on floor surfaces in 2nd floor food preparation areas, warewash and dry storage areas. Clean and sanitize where needed. -Grimes observed under cooking equipment. -Black residue on walls around 3 bay warewash sinks.
