45 The following observation were noted:
- Damaged garbage grinder observed in the prep area.
- Damaged reach-in refrigerator observed in the storage area.
49 A backflow prevention device (faucet with vacuum breaker or hose Bibb) was not observed on the mop sink.

1 The City of Philadelphia Food Safety Certified person has the certification expired from 7/23/2023.
6 Food employee did not follow proper hand wash procedures. Food employees did not wash hands between changing tasks.
8 Clogged hand wash sink drain observed in the prep area.
13 Cookie dough observed on several tables without cover.
14 Baking equipment (trays) observed with food debris and two baking trays observed with mouse droppings.
**The food-contact surfaces of cooking and baking equipment shall be cleaned at least every 24 hours
14 Soiled prep tables and counter tops observed throughout the facility.
14 - The ware wash ( 3-compartment) sink not clean to sight and not properly setup for sanitization of multi-use utensils and food equipment; Debris present in all bays of the 3-compartment sink.
*Sanitarian instructed the person in charge about the wash, rinse, and sanitize procedure.
34 No thermometer observed in the reach-in refrigerator.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 Visible physical evidence of rodent (mouse droppings) observed on one of the prep table, on two baking trays and on the floor perimeters throughout the facility (first floor and basement area).
36 -Several insect control devices (fly glue strips with flies) located in ware washing /prep area (over a prep table)) with potential to contaminate food, equipment, and/or utensils.
37 Food (buckets)is not stored at least 6 inches above the floor in the facility.
45 -Non-NSF or ANSI approved refrigerator/freezer in use.
-The commercial refrigerator is not operating.
-The closed display case and walk-in cooler not in use during inspection.
46 -Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration.
47 - Soiled cooking equipment observed throughout the facility.
**The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
47 Flours and other residues observed on tables, counter tops, and baking equipment.
49 Clogged hand wash drain in the prep area.
51 Water ceiling tiles observed in the restroom area.
53 -Clutter observed in the side of the facility (toward the bathroom).
-Unused equipment and items not necessary for the bakery daily operation observed in the basement.
-Debris and excessive amount of mouse droppings observed on the floor throughout the facility.
53 Cracked floor tiles in the ware washing area.
Dirty walls in the prep area.
Holes and missing ceiling tiles observed in the ware washing/prep area.
Water ceiling tiles observed in the facility.
53 Mop observed in the bucket in the ware washing area.
54 Dust accumulation observed on the exhaust hood.
