49 Leak observed from the plumbing under the mechanical warewashing machine.
52 Dumpsters observed with open lids in the rear refuse area; receptacles or waste handling units should be inaccessible to insects and rodents.

- Dead cockroaches observed on the floor in the basement utility room.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
**The Person in Charge arrived during the inspection.
14 The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher.
**Temperature measured at 120 degrees F. Facility will manually sanitize all food-contact surfaces until the machine is fixed.
21 Bulk containers of sauces prepared in the food facility, located in the walk-in cooler, were not date marked.
**Items were dated during the inspection.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the warewash area.
**Items were removed and placed in an area that prevents contamination.
