4 Employee's beverage containers (water bottles) were observed in a food preparation area on top of covered prep sink and a prep table. They were discarded. 46.151(a) - Food Contamination Prevention: Eating, Drinking or Using Tobacco (a) Eating, drinking or using tobacco. (1) General prohibition. An employee may not eat, drink or use any form of tobacco unless the use occurs in designated areas where the contamination of exposed food; clean equipment, utensils and linens; unwrapped single-service and single-use articles; or other items needing protection cannot result. The sole exception to this prohibition is in paragraph (2). (2) Exception for closed beverage containers. A food employee may drink from a closed beverage container if the container is handled to prevent contamination of the following: (i) The employee’s hands. (ii) The container. (iii) Exposed food; clean equipment, utensils and linens; and unwrapped single-service and single-use articles.
13 The indirectly drained food prep sink is in poor repair and not fit to be used to wash produce. Presently, employees are using a bucket to wash tomatoes rather than the food prep sink. Note: Whole onions observed and are also sliced in this restaurant.
14 -Dried residue on inner upper surfaces of the refrigerated soft serve machine. Person in charge stated that the machine would be cleaned later in the day. -Tan slime observed on the interior of the ice machine bin around the ice inlet cover. The person in charge emptied, washed, rinsed, and sanitized the ice machine before further use.
45 -The food prep sink is not in good repair and is not being used for washing produce. -One of two microwaves is not working in the food preparation area.
53 -Static dust on backflow preventer and water pipes near the mop sink. -Oil stained cardboard on floor perimeters in the fryer area. -Water and ice on the floor around the ice machine.

14 -Onion dicer has an accumulation of dried food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. -Residue on inner upper surfaces of the soft serve machine.
45 Tape on refrigerator surfaces is not smooth and easily cleanable.
45 -The food prep sink is not in good repair and is not being used for washing produce. -One inoperable under counter refrigerator observed in the food preparation area.
