2 Food facility does not have a Health Awareness Reporting Procedure for food employees.
Corrective Action: A Health Awareness Reporting Procedure policy was distributed and the facility was advised to hang the procedure for all food employees to see.
11 One dented canned item observed in the basement area.
Corrective Action: The can was segregated and the PIC intends to return it to the distributor.
13 -Observed ready-to-eat cooked bacon stored in the reach in refrigerator in the prep area not covered or protected from contamination.
** Advised to provide coverings for all cooked foods present in the facility.
-Observed raw shell eggs stored above ready-to-eat lettuce in the reach-in refrigerator in the prep area.
-Observed raw fish stored directly above raw beef above ready to eat foods in the reach-in refrigerator in the prep area.
-Observed ready-to-eat deli meats stored on the same shelf as raw shell eggs in the reach-in refrigerator in the basement.
** Advised to ensure the following organization is present to prevent contamination.
* ready to eat foods including cooked foods, breads, fruits, and vegetables.
* raw fish
* raw pork
* raw whole beef
* raw ground beef
* raw chicken and raw shell eggs
14 -Black and brown residue observed on the interior of the ice machine bin around the back vertical tray.
** Advised to remove / melt the ice present and clean the interior of the ice machine.
-Observed a stapler stored in direct contact with the cutting board at the cold prep refrigerator.
** The cutting board was sanitized upon request.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 150 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions.
Corrective Action: The sanitizer was remade and it retested at 300 ppm.
35 Repackaged foods present in the facility are not labeled with the common name of the food for easy identification.
37 -Observed the coffee station at the front service counter without a sneeze guard or protection from patron contamination.
-Observed bottled beverages stored on the floor in the patron area instead of stored at least 6 inches above the floor.

-Debris observed on the lower shelf of the prep tables.

-White debris / power observed on the shelf above the cold prep refrigerator.
-Residue, food debris and grime observed on the interior and exterior of the cold prep refrigerator, and reach-in refrigerators and freezers.
-Observed ice accumulation with debris on the interior walls of the chest freezers.
52 Observed trash and cardboard boxes stored on the floor in the basement area instead of securely inside a waste receptacle.

-Observed a glue trap stuck to the wall in the basement area.

-Observed a spilled over bag of a dried food on the floor in the basement in front of the dry storage shelves.
-Grease accumulation and debris observed on the floor beneath the deep fryer.
-Grease and debris observed on the floor beneath the flat top grill and in the corner of the hot prep area.
53 Observed a mop stored in a mop bucket instead of elevated to promote air drying.
53 -Damaged floors, walls, and ceilings observed in the basement area.
-Grease stained ceiling tiles present in the prep area.
-Observed exposed sheet rock present as the walls and ceilings in the basement area.
