1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. Person in-charge does not know the names of foodbourne illnesses. Inspector reviewed in detail.
2 Food facility does not have a Health Awareness Reporting Procedure for food employees. Inspector will send a sample copy with report.
8 Soap was not available at the handwash sink in the sales area. Soap was provided.
53 Mop head was observed improperly stored.
54 Clean vent in the toilet room area.

8 Paper towel dispenser empty at the handwashing sink in the food retail area. **PIC took corrective action and provided paper towel during time of inspection.
36 An unapproved insect control device observed located on same shelf with food in the food retail area with potential to contaminate food, equipment, and / or utensils. **PIC took corrective action and discarded unapproved insect control device during time of inspection.
41 Food dispensing utensil in the bulk ingredient containers observed in direct contact with food and not having handle stored above the top of the food.
45 The following were observed: -Damaged doors on cabinetry units behind front service counter -Damaged kick plate on one reach-in refrigeration unit in food retail area
47 The following were observed: -Food debris on lower interior surfaces of reach-in refrigeration units -Ice accumulation on interior surfaces of chest freezer -Static dust and debris on lower interior surfaces of cabinetry units
