Milk crates were used for elevation throughout the facility. Wooden shelving was observed in the basement, and a wooden plank was observed under the deli slicer. Fuzzy green liner was observed on shelves in the retail area. Manufacturer's wrapping paper observed on equipment.

14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was greater than 400 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
Note: The person in charge diluted the concentration of the sanitizer to 200-300 ppm.
