13 Uncovered food was observed in walk-in cooler and bottom of cold holding unit. All food must be covered prior to storing.
21 Cooked and cut potentially hazardous ready to eat food(shrimp, vegetable and spring roll) prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, were not date marked with the dates they were cook/cut.
36 Visible physical evidence of rodent activity observed.
Note: Mouse feces was observed in the following areas:
*In storage area where bulk food containers are stored and on basement stairs. cleaning needed.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
39 Moist wiping cloths observed lying on table and wok and not stored in sanitizing solution.
45 Raw wood food shelving and crates observed being used in rear storage area. Garbage grinder is installed on a wall mounted shallow hand sink.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code.
Note: Food safety certified person could not name the five reportable foodborne illnesses and their symptoms. The name and symptoms of the five reportable foodborne illnesses(salmonella, E. coli, Norovirus, hepatitis A and shigella) and symptom(vomiting, diarrhea, fever with sore throat, jaundice, boils on the skin and mouth..) was given to PIC.
8 The handwash sink in the food prep area was blocked by bucket and dishes and not accessible at all times for employee use.
Note: Items were removed from blocking the hand sink at time of inspection.

Note: Hand wash sink must not be used for anything other than hand washing. Sink was emptied and cleaned at time of inspection.
