1 No Philadelphia Food Safety Certificate observed. ServSafe observed. Corrective action: Health Sanitarian emailed person in charge application on 2/13/2024. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
4 An employee's Food container was observed in a under-counter refrigeration unit in the food preparation area. Corrective action: Person in charge removed food container during the time of inspection.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. Corrective action: Person in charge remade quarternary ammonia concentration in the sanitizing during the time of inspection and was measured between 200-400 ppm.

53 The following observations were noted: -Debris along floor perimeter under shelving in the rear storage and under the 3-basin sink. -Debris along floor perimeter of food prep area. -Static dust accumulation present on fan cover of ice machine.
54 Employee clothing observed on shelving with single-service items in the rear storage area. Corrective action: Employee's removed clothing items during the time of inspection.
