1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
6 No hot water observed at the handwash sink on the mobile unit. Water was observed disconnected.
9 Vendor lacks support facility/commissary documentation.
36 Rear outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
36 Sticky fly trap observed over top of prep area where dicer and juicer is located.
37 Food that was displayed on top of the grill was observed without a sneeze guard to protect from contamination.
37 Food is not stored at least 6 inches above the floor.
45 Non-ANSI/NSF sanitation approved juicer observed.
47 Sticky trap full with dead flies observed on a box under the prep table.
59 Prior to the start of renovations a completed Plan Review Application with proper plans, drawn to scale, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the Office of Food Protection. Plan review application must be submitted and approved prior to any food operations.
