11 Several pints/gallons of milk observed with "Sell By" dates of 11/24, 12/4,/12/8, and 12/10 in walk-in cooler and reach-in refrigeration unit near tray line. **PIC was instructed to discard all milks during time of inspection.
13 Raw pork observed stored above raw fish in walk-in cooler. **PIC was instructed to store raw fish above raw pork during time of inspection.

26 Chemicals were stored on the same shelf with single service articles in the food prep area. **PIC was instructed to move chemicals to a chemical only designated area during time of inspection.
37 Pipe to condenser in walk-in cooler observed leaking directly over raw pork and fish. **Foods storage: prohibited areas. Food may not be stored in any of the following locations: Under a leaking water line (including a leaking automatic fire sprinkler head), or under a line on which water has condensed. **PIC was instructed to move food away from leaking condenser pipe during time of inspection.
