1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Servsafe observed during the inspection. Application for city cert was sent via email.
8 The following was observed: -Paper towel dispenser empty at the handwash sinks in both bar area. -Old food residue, dishes and utensils observed in the handwash sink. **Person in charge took corrective action
14 Soda guns in both bar areas observed with old residue. **Person in charge took corrective action
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients.(RIce)
42 Customer self-service utensils being stored in a manner that hand contact cannot be avoided with the mouth / lip surface of the utensil.

1 Food Safety Certified person was observed without the City Certificate. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **An application for the City Certificate was issued to Food Safety Certified person during time of inspection.
8 The handwashing sink in the front food prep area was blocked by food storage containers and not accessible at all times for employee use. **PIC took corrective action and moved food storage containers to designated area during time of inspection.
8 Paper towel dispenser empty at the handwashing sink in the front food prep area. **PIC took corrective action and provided paper towel at handwashing sink during time of inspection.
8 Residue observed around faucet handles of all handwashing sinks in the food prep area. *PIC took corrective action and properly washed, rinsed, and sanitized exterior surfaces of faucet handles during time of inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 Outer opening on lower surface of rear door in the food facility does not protect against the entry of insects, rodents, and other animals. **PIC took corrective action and put in a work order for maintenance to service lower surface of door during time of inspection.
36 An insect control device located above dish rack in rear food prep area with potential to contaminate food, equipment, and / or utensils. **PIC took corrective action and moved dishrack to another location in the food prep area away from insect control device during time of inspection.
41 Common bowls and scoopers observed without handles in direct contact with bulk food ingredients. **PIC took corrective action and removed bowls and scoopers from bulk ingredient containers during time of inspection.
45 The following were observed: -Milk crates being used for elevation of food in food prep area which are not smooth and easily cleanable -Bread crates being used for elevation of food in walk-in cooler/freezer which are not smooth and easily cleanable
45 Cabinetry units on hand-wash/coffee station in rear food prep area observed damaged.
47 The following were observed: -Food debris/food residue around perimeters of floors in walk-in coolers/freezer -Static dust accumulation on fan guards in beer walk-in cooler -Food debris/grease accumulation on interior surfaces cooking/warming equipment -Food debris on lower interior surfaces of reach-in refrigeration unit/freezer -Ice accumulation on interior surfaces of chest freezer
51 The following were observed: -Residue around perimeters of toilet in unisex employee restroom -Static dust accumulation on air vent in unisex employee restroom
51 Unisex employee restroom observed without receptacle for feminine sanitary napkins.
53 Static dust, debris, and cobb webs observed around perimeters of floor throughout front and rear food prep areas.
53 Mop observed stored in bucket and not hanging in a manner that facilitates air drying.
53 Brooms and dustpans observed stored on floor and not elevated on a utility rack designated for maintenance tools.
54 Hood filters have an accumulation of static dust and grease.
54 The following were observed: -Dull lighting in beer walk-in cooler -1 light out under exhaust hood in food prep area
