8 - Soap was not available at the handwashing sinks in the restroom area and behind the register.
- Paper towel dispenser empty at the handwashing sink at the register area.
**Soap and paper towels were supplied the to handwashing sinks at the time of the reinspection.
13 Observed the handle of the ice scooper in contact with the ice in the ice machine.
**The ice scooper was removed from the ice machine at the time of the reinspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0-100 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
**The sanitizing compartment sink was emptied out and refilled at the time of the reinspection.
35 Working bulk food ingredient storage container (sugar) was not labeled with the common name of the food.
47 Observed napkins and a lid in the catch basin of the drain under the coffee station.
**The drain was cleaned out at the time of the reinspection.
49 Observed the main water pipe with a leak.
53 Observed an old wet paper towel underneath the prep table in the warewash area.
**The person in charge cleaned under the prep table at the time of the reinspection.
53 Observed a broom and dust pan idle near the reach-in refrigeration unit.
**Broom and dustpan was relocated to the utility area at the time of the reinspection.

47 Nonfood contact surfaces: dusty fanguard observed in 1 under-counter refrigerator.
52 Overfilled dumpsters and scattered trash bags observed in exterior storage area.

6 Food employees did not follow proper handwashing procedure, due to lack of hot water. *PIC corrected by adjusting hot water heater.

13 Donuts were not protected from contamination by flies and fruit flies: donuts observed with flies landing on their surface. *PIC discarded all product.
36 Visible physical evidence of insect activity observed throughout: flies and fruit flies observed at food/beverage service counter, drive-through window area, rear warewashing area and restroom.
41 Ice & sugar scoops observed in direct contact with food, and not having handle stored above the top of the food.

53 Premises: dusty ceiling/wall vents observed in ordering area & cooking area; floor needs general cleaning under front counter & under equipment; dusty ceiling tile behind front counter.
