14 There was no sanitizer readily available at one of the bar warewash machines.
Sanitizer was provided.
34 Some beverage box refrigerators were lacking thermometers in the bar area.
Thermometers were placed in these units.
45 Water is draining into and collecting in two beverage boxes in the bar area. Rust observed on the lower surfaces of these units.
45 Wooden pallets used to elevate items in the walk in cooler rather than NSF approved dunnage racks.
47 -White residue observed on the cover of the ice inlet in the ice machine.
-White residue observed on the outer surfaces of the warewash machine in the kitchen.
-Tan residue observed on the outer surfaces of the warewash machines in the bar area.
-Accumulated oil observed in the lower fryer cabinets.
49 A mop sink was not observed.
There is a drain in the parking lot near the back door.
51 -The employee toilet room door is not self closing.
-Women's toilet room - One of three stalls does not have a door.
-Men's toilet room - Three of six stalls do not have doors.

-Women's - Soap dispenser is making a loud sound when in use.
-Static dust on the exhaust vent cover in some toilet rooms.

-Static dust on wall in the kitchen area.
-Static dust on the ceiling surfaces in the rear seating area.
-Static dust on a vent cover over the door in the rear storage area.
-General dust and debris on the floor perimeters in the bar area.
53 The brooms and dust pans are stored on the floor rather than hanging on utility racks in the bar and kitchen areas.

14 Black residue and pink slime observed on the bar gun beverage contact surfaces.
The gun was cleaned and sanitized during the inspection.

45 Kitchen prep and hand sink are not sealed to the wall.
45 Water is draining into and collecting in a cold prep refrigerator in the kitchen.

11 5lb bag of shredded cheese observed expired March 2022.
The person in charge discarded the expired item.
14 The QAT chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0-100 ppm, and not at least 200 ppm as required.
The person in charge primed the dishwashing machine's sanitizer levels and the sanitizer reached 200ppm during time of inspection.
21 Opened commercially processed ready to eat food (Cheese, cooked chicken, and cooked bacon bits), located in the reach in, and held more than 48 hours, was not marked with the date it was opened.
35 Working bulk food ingredients (Flour, Rice) storage containers are not labeled with the common name of the food.
The person in charge labeled the bulk items.
39 Moist wiping cloths observed lying underneath the cutting board in the food prep area and not stored in sanitizing solution.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
45 Kick plate missing from reach in refrigerator in the food prep area.
46 Food facility lacks sanitizer test strips to monitor chlorine or QAT sanitizer concentration.
59 A second hood was installed in the food preparation area without plans being sent to plan review. Prior to the start of renovations a completed Plan Review Application with proper plans, drawn to scale, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the Office of Food Protection.
