13 A large container of lemonade was stored uncovered in the walk in refrigerator. A cover was provided.
14 Black residue observed on the interior of the ice machine bin around the water inlet area. The person in charge agreed to turn off the ice machine, empty it, wash, rinse, and sanitize all surfaces. The surfaces should be allowed to air dry before further use.
36 Visible physical evidence of rodent (mouse droppings)/ insect (flies) activity observed in the service / prep and storage areas.
37 Boxes of food is not stored at least 6 inches above the floor in the walk in refrigerator.
47 Observations: - Mouse droppings on the lower shelf surface under the counter. - Flies landing on surfaces in the rear prep area. - Dust and debris on the outer surface of the rice bulk storage container. - Residue observed on walk in freezer door. - Accumulated ice on walk in freezer surfaces. - Residue on surfaces in the prep area.
49 Backflow prevention devices were not observed at the water meter, water filter or ice machine.
51 Both the employee and customer bathrooms are being used for storage of restaurant equipment (dunnage rack and other items) that prevent the proper use of these areas. - Pooled water on the customer toilet room floor. The water was mopped up in the customer toilet room.
52 The used cooking oil dumpster and recycleables dumpster were observed open when not in use rather than tightly closed. The lids were closed during the inspection.
53 -Missing ceiling tiles observed in the rear storage area. -Wood on ceiling surface in the rear storage area. -Openings in wall surfaces.
53 Observations: -Mouse droppings observed on floor perimeters in the rear storage area near the rear door and behind equipment. -Water on the floor in the rear storage area near the walk in freezer door. -Accumulated oil residue on floor perimeters in the fryer area. -General debris on the floor perimeters in the rear near the door.
53 The brooms and dust pans are stored on the floor rather than hanging on a utility rack when not in use.

4 Employees' bottles of water observed in a food preparation area. The bottles observed on the prep surface were discarded. Other bottles were observed stored in a holder on the wall. Inspector requested that employees drink from bottles away from the prep and service areas.
8 Observed chairs blocking the hall to the bathrooms. A sign on the wall near the hall stated "No restroom available Employees only Thank you!!!!" Customers are being denied access to the bathroom. This restaurant has a sit down eating license. Therefore, customers are required to have access to the bathroom for hand washing. Also, the chairs were observed stacked on the tables. At least one handwashing sink and a number of handwashing sinks necessary for the convenient use by consumers shall be provided in food facilities which allow consumption of food on the food facility premises. Handwash sinks for consumers shall be located in easily accessible areas and as required in ยง 46.824(a)(2). Inspector requested that the bathroom be cleaned out and made accessible to paying customers. An employee began removing all of the items stored in the bathroom. Inspector also requested that seating be made accessible to the paying customers.
8 The hand sink in the front service area did not provide water at 100 degrees F. The water was probed at 71 degrees F maximum. Also, this sink is leaking at the pipe below.
8 There was no trash can at the rear prep area hand sink. A can was placed at this sink.
14 Wiping cloths observed stored in sanitizer bucket with sanitizer concentration of 0 ppm. A fresh solution was prepared. The solution was tested at 200 ppm.
41 Scoops observed stored in stagnant water. The water in the dip wells were turned on to run during the inspection.
45 Equipment is not in good repair. -The service area hand sink is leaking at the pipe below and lacking water at 100 degrees F. -The hot air dryer in the employee bathroom is not working.
