-observed raw wood shelving used in rear storage area

-dust accumulation observed underneath storage area shelves

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
*Sanitarian emailed PIC directions on how to obtain a city certificate*
35 Prepackaged prepared food items are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 Visible physical evidence of rodent activity observed: mouse droppings and rat droppings observed in the warewashing area and prep areas, and throughout floor perimeters of ice cream serving area.
36 Visible physical evidence of insect activity observed: heavy fruit fly activity observed in the front prep area, concentrated on the box of bananas.
37 Food is not stored at least 6 inches above the floor in the retail area

-mouse droppings observed on lower level of shelving in the prep area
53 General cleaning needed: along floor perimeters in the warewashing area, prep area and ice cream area where mouse and rat droppings were observed.

8 No sign or poster posted at the handwashing sink in the toilet room to remind food employees to wash their hands.
**Person in charge posted a reminder sign during the inspection.
47 Accumulation of ice on the interior surfaces of both chest freezers in the rear storage area.
49 Facility does not have a utility sink.
52 Observed the dumpsters outside with the lids open.
**Person in charge closed the lids during the inspection.
53 Premises cleanability:
- Some floor tiles are cracked in the retail area.
- Some ceiling tiles are damaged in the retail and rear storage area.
- Large square hole in the wall in the rear storage room.
53 Maintenance tools (brooms/dustpans) observed stored on the floor in the rear storage area instead of being elevated in a designated location.
53 Unused ventilation hood and small convection oven in the rear storage area.
53 Cleaning of dust and debris along the floor perimeters of the service/cash register area.
54 Lights throughout the facility are not shielded from breakage.
