53 The following observations were noted: -Open wall seams in the food prep and basement areas. -Damaged ceilings in the food prep and basement areas. -Unsealed gaps and holes in the food prep and basement areas. -Damaged floor tiles in the food prep and basement areas.

4 An employee's beverage containers was observed in a reach-in refrigeration unit. Corrective action: Person in charge removed employee beverages during time of inspection.
13 The following observations were noted: -Knives being stored between wall surface and warewashing sink in the basement area. Corrective action: Person in charge removed knives from crevice during time of inspection. -Spoons and scoopers observed submerged in food items in working bulk containers. Corrective action: Person in charge removed scoopers and spoons from working bulk containers during time of inspection.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the food prep area. Corrective action: Person in charge relocated chemicals during time of inspection.
26 Observed rodenticides not labeled by the manufacturer for food facility usage, in the basement area.
36 Visible physical evidence of rodent activity (Mouse droppings) observed in the storage area in the food prep area. *Health Sanitarian advised person in charge to call exterminator*
