35 -Repackaged cut fruit, desserts, and cupcakes are not labeled properly with the ingredient statement, net weight, distributed by statement and/or nutritional facts. -Prepared salads are lacking the net weight on the label.
37 -Packaged candies are not elevated at least 6 inches above the floor in the retail area. * Food should be stored at least 15 cm (6 inches) off of the floor
45 -Plastic crates used to elevate items rather than NSF approved dunnage racks in the rear and basement storage areas. -Cardboard liners in the retail area are not easily cleaned -Artificial grass liners in the closed deli display case are not smooth and easily cleaned and block air flow.

13 The designated sink for food prep appears to be directly plumbed rather than indirectly plumbed with a catch basin. Whole produce is rinsed in this store. For example, tomatoes, lettuce, watermelon, pineapples, strawberries and other fruits and produce that are cut and packaged for sale in this store. Possible Solutions: -Purchase the produce and fruit pre-rinsed and provide documentation that it was purchased that way. -Have the designated sink indirectly plumbed with a catch basin that is accessible for inspection and cleaning.
26 Chemicals (lighter fluid) was stored on the same shelf with food (bottled spring water) in the retail area. An employee moved the bottles of spring water.
26 Observed insecticides (Black Jack insect spray and Combat bait traps) or rodenticides (Tom Cat Mouse bait) not labeled by the manufacturer for food facility usage, in the basement storage and toilet room areas. The observed items were removed from the store.

9 Food Facility Person in Charge not able to provide documentation that Tres Leches Cakes are from an approved source. The person in charge removed the cakes from the retail refrigerator to be returned to distributor.
13 There is no designated food prep sink for the rinsing of tomatoes, lettuce, watermelon, pineapples, strawberries and other fruits and produce that are cut and packaged for sale in this store. Stop all rinsing of produce and fruit in the 3 basin sink. Possible Solutions: Purchase the produce and fruit pre-rinsed and provide documentation that it was purchased that way. -Install the food prep sink.

14 The person in charge set up the 3 basin sink for sanitizing and washing. Only the first basin was observed with a rag and water at the beginning of the inspection. The inspector explained the proper set up of washing, rinsing, and sanitizing. The person in charge then set up the sink properly.
21 Cut fruit, potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the retail refrigerator and closed display case, was not date marked. Repackaged desserts, opened commercially processed ready to eat food, located in the retail refrigerator and closed display case, and held more than 48 hours, was not marked with the date it was opened. The person in charge began placing dates on the packages.
