14 Deli slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
*Person in charge dismantled and cleaned slicer.
21 Potentially hazardous ready-to-eat food prepared in the food facility (Chicken) and stored in the refrigeration units was not date marked.
Note: The person in charge date marked the appropriate food.
35 The food in the bulk containers were not labeled with their common use names.
43 Utensils observed not stored inverted in the kitchen storage area.
51 Restroom areas lacks a receptacle with lid or cover for sanitary napkins.
53 -Water stained ceiling tile observed in the utility room.
-Gaps observed on the ceiling tiles in the storage area.
-Porous ceiling tiles observed in the ware washing/dish machine area which are not easily cleanable.
53 Old mouse droppings observed behind water heater in utility closet.
53 Brooms observed on direct floor surfaces in utility closet.
54 Rusty cover ventilation observed inside walk-in cooler.

-One light observed burnt out underneath the ventilation hood in the kitchen food prep area.
