1 Food safety manager does not have Philadelphia Food Safety Certificate - application sent via email.
1 Food safety manager was not initially present at the food facility - arrived during inspection.
4 An open employee's beverage container was observed in a food preparation area.
6 Food employees observed donning single use gloves without a prior hand wash.
8 The handwash sinks in the bar and food prep areas were blocked by utensils and not accessible at all times for employee use - items removed.
8 1. Paper towels were not available at the hand sink in the bar area - items provided during inspection.
2. Hand soap was lacking at the hand sink in the food prep area - provided during inspection.
13 Several raw salmon were stored above cooked shrimp in the walk-in cooler.
14 Black residue observed on the interior surfaces of the soda gun nozzles and holsters.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was below 50ppm, and not 50-100 ppm as required - discontinue use until repaired, must manually wash dishes and utensils.
27 Food facility preparing foods using reduced oxygen packaging without the required written procedures or HACCP plan - discontinue vacuum sealing foods until a HACCP plan is submitted and approved.
36 Visible physical evidence of rodent activity (rat droppings) observed throughout the basement area.
37 Facility lacks a separation between ice used for beverages and ice used as a coolant for equipment behind the bar.

2. Sink behind the bar area is lacking a handle for the hot water.
3. Multiple defective pieces of equipment in the food prep and basement areas - cold prep unit, under counter refrigerator (used for dry storage), reach-in refrigerator, and ice machine (used for towel storage).
45 Handsink, food prep sink, and garbage grinder sink in the food prep area are not properly sealed to the wall.
46 1. Warewash sink behind the bar is lacking a handle for the hot water - wrench observed attached to the knob.
2. Detergent and sanitizer dispenser system used for the warewash sink in the basement is not effectively dispensing cleaning/sanitizing agents.
46 Food facility lacks sanitizer test strips to monitor quaternary ammonium sanitizer concentration.
46 Thermometer on the mechanical dishwasher was not operating during inspection.
47 Exhaust hood filters and interior surfaces of the fryer cabinets have an accumulation of grease.
47 Chest freezer in the food prep area has an accumulation of ice on interior surfaces.
49 1. Sink behind the bar area is lacking a handle for the hot water.
2. Leak observed from the drainpipe under handsink in the basement.
50 Indirect waste lines observed draining into a trash/recycling container instead of an approved utility sink or floor drain.
51 Toilet room lacks a separate lidded receptacle for the disposal of sanitary products.
52 Trash receptacles observed open, not covered with tight-fitting lids.
52 An adequate amount of receptacles for refuse and recycling was not available - trash bags observed stored outside of receptacles.
53 1. Debris observed on the floors under and behind shelving and equipment throughout the facility.
2. Rat droppings observed on the basement floor near carbon dioxide tanks and in storage areas.
3. Walls in the food prep area are grease stained.
4. Items unnecessary to the daily functions of the facility observed stored/cluttered in basement, creating a potential harborage area.
53 Base coving is lacking along the floor-wall junctions in areas throughout the food prep area.
53 Wood used for base coving in the basement warewashing area is not smooth, easily cleanable, and nonabsorbent.
54 Inadequate lighting is available in the cold prep unit refrigerator and under the exhaust hood in the food prep area.
Note: The light intensity in a food facility shall be at least 220 lux (20 foot candles) inside equipment such as reach-in and under-counter refrigerators, and at least 540 lux (50 foot candles) at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders or saws where employee safety is a factor.
62 Before engaging in an activity which requires a HACCP plan, a food facility applicant or operator shall submit a properly prepared HACCP plan to the Department for approval for vacuum sealing.

11 Expired half & half was obsreved in the walk-in cooler dated, 11/1/2023. The person in charge discarded the expired half & half.
42 Plates and bowls were not properly inverted in the food preparation area.
45 Concrete blocks, which are not smooth and easily cleanable, were being used to support equipment in the basement area.
45 The following observations were noted:
-An unused ice machine in the basement storage area.
-The hot water handle of the 3-compatmnet sink located in the bar was in need of general repair.
47 Rust around shelves were obsreved in the walk-in coolers.
51 The air vent located in the women's restroom area was in need of cleaning due to dust accumulation. The person in charge cleaned the dust.
