6 Observed food employee using personal phone with gloves on before preparing hot chocolate without changing gloves. Corrected on Site: Food employee was instructed to remove gloves and wash hands.
8 Soap was not available at the handwash sink in the warewash area. Corrected on Site: Soap was added upon request.
11 Observed the following: - One expired Gerber pear juice - Four dented cans - Three expired Gerber baby food jars Corrected on Site: All items were disposed of upon request.
20 Observed the following: - Foods inside of cold prep refrigerator (tomatoes, mayo, etc.) between 43-45F -Foods inside of open display case (cheese) at 50F Corrected on Site: Cold prep refrigerator temperature was lowered to 32F and cheese was moved to closed display case and was lowered to 40F.
21 Observed deli meats and cheeses, prepared in the food facility and held for more than 48 hours, located in the closed display case, was not date marked. Corrected on Site: All foods were date marked upon request.
45 Observed the following: - Tomatoes observed stored inside cardboard box inside cold prep refrigerator. - Cardboard lining observed under chips in retail area
47 Observed grease accumulations on exhaust hood.

53 Observed the following: -Floor and walls around grill observed soiled with dirt and debris. Cleaning is needed. -Basement is very cluttered, organization is needed.

13 Multiple food items observed uncovered in closed display case and inside reach-in freezer. Food items were covered with lids and foil during the inspection.
37 Boxes of ketchup and mayonnaise observed on floor in food prep area. All food items should be stored 6 inches above the floor.
39 Moist wiping cloths observed lying on food prep counter and not stored in sanitizing solution.
45 - Tomatoes observed stored inside cardboard box inside cold prep refrigerator. - Cardboard lining observed under fresh sliced fruit and cakes inside reach-in cooler.
45 - Non-NSF/ANSI equipment in use - chest freezers, and hot plate. - 3 basin sink observed with small leak when water is draining. Water is draining into a container.
47 - Ice accumulation observed inside chest freezer.
53 Brooms and dust pans observed on floor and not elevated when not in use.
53 Floor behind grill observed soiled with dirt and debris. Cleaning is needed.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
21 Deli meat and cheese opened commercially processed ready to eat food, located in the closed display case, and held more than 48 hours, was not marked with the date it was opened.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the food prep area.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible.
