1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. *City certified food manager (owner) arrived later.
8 No sign or poster posted at the handwash sink in the men's restroom to remind food employees to wash their hands. *PIC hung a sign
21 Potentially hazardous ready to eat food (dairy-based cake slices) prepared in the food facility and held for more than 48 hours were not date marked. *PIC provided date labels
37 Beverage cases were not stored at least 6 inches off of the floor in walk-in cooler.
45 Equipment repair/adjustment: - cardboard floor liner observed on the floor in the walk-in cooler - Garbage grinder attached under shallow handwashing sink in prep area -One domestic style (non-NSF or ANSI-Sanitation approved) freezer observed in the basement.
45 Wood cutting board observed at cooking line does does not meet NSF or ANSI-Sanitation standards.
47 Equipment needs cleaning: dusty fanguards in the beer bottle/dessert refrigerator and walk-in cooler
51 Restrooms: dusty ceiling vent/fan cover observed in men's restroom.
52 Floor of inside trash storage room needs general cleaning.
53 Broom/mops observed stored on the floor (instead of being hung/elevated when not in use).
53 Premises: Floor perimeter under kitchen sinks needs general cleaning; liquid observed on basement floor to be dried. Older mouse feces on basement floor needs cleaning.
53 Cleanability of surfaces: * Porous ceiling tile in kitchen is not smooth & easily cleanable: in cooking area. * Open space observed in between exhaust hood and ceiling tile (exposed due to utility line penetrations). * Wood floor in side storage room (south-side) is not smooth and easily cleanable.

13 Mildew stains observed in condenser/condenation drip pan area inside of ice maker. *PIC took unit out of service and switched to bagged ice that was on site.
45 Floor of walk-in cooler is not smooth & easily cleanable.
45 Equipment repair/adjustment: - Garbage grinder attached under shallow handwashing sink in prep area -Two domestic (non NSF or ANSI-Sanitation approved) freezers observed in basement
47 Equipment needs cleaning: fanguards & shelving in the beer bottle/dessert refrigerator; dusty lids on containers used to store take-out containers in basement; dust accumulation on the fanguard in the walk-in cooler
49 Plumbing systems: ill-fitting sump pump cover observed in basement
51 Restrooms: Floor/wall junctures in the ladies restroom are not coved; damaged coving observed in the men's room.
52 Open dumpsters observed in trash storage room (to be maintained closed). Miscellaneous items stored on floor in trash storage room.
53 Floor perimeter under bar needs general cleaning; liquid observed on basement floor.
53 Indoor surfaces: Acoustic/porous ceiling tiles observed in the kitchen are not smooth and easily cleanable; cracked floor tile observed in the food preparation area; floor/wall junctures are not smoothly coved in food preparation area.
53 Broom was stored on the basement floor (to be hung/elevated when not in use).
