14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. PIC replaced sanitizing solution at the time of the inspection.
36 Visible physical evidence of insect activity observed in the establishment. - Small flies observed on the walls near the drying rack and windows in the diswasher area - Small flies observed on the drying rack in the warewash area - House flies observed in the prep area and warewash area

14 - Black residue, pink slime observed on the interior of the ice machine bin. Ice was discarded and bin was wash, rinse and sanitized during the inspection. - The can opener had an accumulation of food residue on the food contact surfaces. PIC relocated the can opener to the warewash area.
14 Accumulation of dried food residue observed on the inside surfaces of the microwave oven.
14 Wiping cloths observed stored in sanitizer bucket with sanitizer concentration of 0 ppm. PIC provided new sanitizer bucket at proper concentration.
21 Opened commercially processed ready to eat food (cheese), located in the walk-in cooer, was not marked with the date it was opened. PIC date marked the food item during the inspection.

45 The door to the kitchen staff office room did not have a handle, a cloth was being used to pull the door open.
47 Dust accumulation observed on the refrigerator power cords hanging from the ceiling in the food preparation area.
50 Condensation of from the ice machine was not properly drained into the nearby indirect drains.
53 Damaged walls, and floors observed in the food preparation area.
53 The floor drain by the ice machine was unclean to sight. Water accumulation obserced around the floor drain.

45 The following observations were noted: - One out of service reach-in refrigerator and reach in freezer. - Front side of the ice machine observed with black tape on the front bottom part / pep area. - The hand wash sink observed unseal to the wall in the kitchen area.
53 The following observatins were noted: - Damaged wall tiles observed in the kitchen area. - Damaged cove base in the kitchen area (near entrance). - Cracked floor tiles near the cooking area. - Missing tiles behind the prep sink (wall) in the prep area. - Peeling paint on the wall in the rear prep area. - Damaged ceiling observed in the ware washing area. - Water stains observed on the ceiling tiles in the ware washing area. - Gaps observed on the ceiling in the kitchen area. - Whole observed on the wall between the kitchen area and prep area.

8 - Delay in hot water temperature at the hand wash sink prep area near the ice machine. The hot water was measured at the beginning of inspection at 90 degrees F, during the inspection hot water was measured at 100 degrees F.
8 Old food residue observed in the handwash sink near the ice machine/prep area. PIC washed, rinsed, and sanitized the handwash sink during inspection.
36 Visible physical evidence of rodent (mouse droppings) observed along the floor perimeters in the warewashing area and dry storage area and insect activity observed throughout the facility (hallway, prep area, and warewashing area).
