14 Black residue, pink slime observed on the interior and exterior of the nozzles for beverage machine dispensers and Condiment machine dispenser in retail area. Corrective action: An employee dismantled, washed, rinsed and sanitized the all nozzles for beverage machine dispensers and Condiment machine dispenser in retail area.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the warewashing area area. Corrective action: Person in charge relocated all chemical products to the bottom shelf prior to completion to inspection.
37 Beverages and boxes of food related items are not stored at least 6 inches above the floor front food prep and inside the walk-in cooler.
45 Chipped and cracked cement flooring inside the walk-in cooler.
49 -Toilet commode in present in the staff restroom area during inspection. -Observed an uninstalled handwash sink in the warewashing area at time of inspection on 2-7-2023.
51 No toilet commode observed inside the staff restroom area during inspection. *Observed an outdoor Portable toilet in front of the facility.

8 Observed paper towel dispenser empty at the handwash sink in the toilet room.
14 Black residue, pink slime observed on the external perimeter around the ice machine dispenser nozzle at the beverage station in the retail area. *Corrective action was taken, person in charge clean and sanitized the ice machine dispenser nozzle at time of inspection.*
19 Food items (hot dogs, Beef Patties) was held between 120 - 129 degrees F rather than 135 degrees F or above as required in the hot holding units. *Corrective action was taken, person in charge increase the heating temperature for the Hot dogs hot-holding unit and discarded the beef patties at time of inspection.*
20 Food items(sandwiches, watermelon cups and mixed fruit cups) was held between 44.4 - 55.9 degrees F rather than 41 degrees F or below as required in the open display case in the retail area.
26 Chemicals observed stored above or on the same shelf with food items in the storage area. *Corrective action was taken, person in charge relocated chemical products to the bottom shelf.*
26 Observed insecticides not labeled by the manufacturer for food facility usage, in the retail area. *Corrective action was taken, the person in charge discarded the insecticide.*
37 Observed bottom chip rack not elevated at this 6 inches off the ground in the retail area.
43 Observed wrapped pizza boxes stored on direct floor surface not elevated at least 6 inches off the ground in the storage room. *Corrective action was taken the person in charge relocated the pizza boxes to the top shelf and above the refrigerator.

45 Observed broken door handle on under-counter refrigeration unit behind the clerk/food prep area.
51 - Observed dust on ceiling vent in the toilet room.
53 The following observations were noted: -Live and dead bugs on glue boards under the reach-in refrigerator and storage rack in the storage room. *Corrective action was taken, person in charge discarded the glue boards at the time of inspection.* -Trash and debris on floor surface under-counter in the clerk/food prep area, under refrigeration units, storage racks throughout. -Wet floor surface near utility sink.
