13 Raw chicken was stored above ready to eat food in the prep area. Raw chicken was relocated.
21 Opened commercially processed ready to eat food (cheese), located in the refrigerator, and held more than 48 hours, was not marked with the date it was opened. Dates were provided.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
45 Foil observed lining surfaces in the prep area.
46 Food facility lacks chlorine sanitizer test strips to monitor chlorine sanitizer concentration.
49 Mop/utility sink was not observed.
51 Dust accumulation observed on the vent cover in the employee restroom.
53 Stained ceiling tiles observed in some classrooms.
53 Brooms and dustpans in classrooms and prep area were stored in direct contact with the floor instead of elevated on a utility rack/hook.
