1 PIC was unable to demonstrate how to properly set up 3 bay ware-washing sink. PIC observed setting up two sanitizer sinks without wash or rinse compartments.
**PIC was instructed of the correct way to set up 3 bay ware-washing sink during time of inspection
1 Food Safety Certified person applied for City Certificate, but application has not been received by the Health Department.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
9 Safe-handling instructions were not observed on pre-packaged raw meats in food prep area.

Meat and poultry that is not a ready-to-eat food and is in a packaged form when it is offered for sale or otherwise offered for consumption shall be labeled to include safe handling instructions as specified in the Food Act

**PIC took corrective action and discarded dented cans during time of inspection.
13 The following were observed:
-Raw chicken stored above uncovered fries and peeled garlic in the reach-in refrigeration unit
-Uncovered potentially hazardous food in reach-in refrigeration
14 The following were observed:
-The quaternary ammonia concentration in the sanitizing solution of the 3-bay ware-wash sink was >400 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
-3-bay ware-washing sink set up incorrectly with two sanitizer compartments.
**PIC was instructed on how to properly set up 3 bay ware-washing sink and to dilute sanitizer concentration to required range during time of inspection.

-Two non-NSF/ANSI drying racks in food prep area
-One non-NSF/ANSI stove in food prep area
-Three non-NSF/ANSI chest freezers in food retail area
-Three non-NSF/ANSI reach-in refrigeration units in food retail area
-One non-operational reach-in refrigeration unit in food retail area (used for dry storage)
-Unused equipment throughout basement area

1 Food Safety Certified person was not present at the start of inspection. Food Safety Certified person observed without the City Certificate upon arrival.

54 Lights in the unisex employee restroom are not shielded from breakage.
