*Person in charge brought soap and paper towels.
11 Observed 4 severely dented (Bush's Baked Beans), swollen, distressed canned items observed in the facilities retail area.
*Person in charge removed cans from shelving unit.
13 -Single-use serving container without handle submerged in food item stored in one container in the food prep area. *Person in charge corrected on site and added scoop with handle.
-Uncovered chicken and potato pancakes observed inside the walk-in cooler.
PIC took corrective action and covered food items during inspection.
14 The following was observed:
-The can opener had an accumulation of food residue on the food contact surfaces.
-Meat slicer with hard old food debris
-Cappuccino machine nasal with hard debris
*Person in charge cleaned listed items and corrected on site.
14 Observed old food residue inside rinse bay of the 3-compartment sink. Corrective action : Person in thoroughly clean to remove old food residue from the rinse bay of the 3-compartment sink prior to completion of inspection.
21 Food potentially hazardous ready to eat food (Chicken and Potato Pancakes), prepared in the food facility and held for more than 48 hours, located in the walk-in refrigerator, was not date marked.
*Person in charge date marked all items and corrected on site.
39 Moist wiping cloths observed lying on kitchen surface area and not stored in sanitizing solution.
45 -Kitchen behind the pizza area not in service.
-Ventilation hood in pizza area not in service.
45 Plastic crates used for elevation of beverages in retail area.

-Cappuccino machine nasal with hard debris
14 -no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use in the pizza department
14 2 sink stoppers observed working (Sanitizer, rinse).
**Wash sink was not holding the water.

45 The following observations were noted:

-Missing ceiling tiles observed in the kitchen area and in the top of the exhaust hood, ware washing area, and dry storage area.

- Clutter observed under the open display case sushi prep area.
- Food debris observed on the floor in the cooking area and sushi area.
- The floor is in need of cleaning in the ware washing area.

8 No sign or poster posted at the hand wash sink in the kitchen area to remind food employees to wash their hands.
**Sanitarian advised the person in charge to provide the sign for the hand wash sink during inspection.
8 - The hot water at the hand wash sink in the sushi area measured at 83 degrees F.
**PIC was instructed to temporarily mark handwashing sink sushi area out of service and to utilize other handwashing sink that reaches peak demand until serviced by maintenance.
14 The following observations were noted:
- Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
- The can openers (2) had an accumulation of food residue on the food contact surfaces.
- The meat slicer observed with food residue and is not being washed, rinsed and sanitized at least every 4 hours.
The PIC took corrective action and washed, rinsed, and sanitized the slicer and can openers during inspection.
21 - No dates on several cooked food and sliced vegetables in the service area and sushi area.
PIC took corrective action and provided dates for all the food during inspection.
36 Visible physical evidence of insect activity (house flies in the service area and fruit flies in the kitchen and ware washing area) observed during inspection.
37 - Boxes with food observed not stored at least 6 inches above the floor in the walk-in freezer.
45 The following observations were noted:
- Domestic microwave Daevo observed in the sushi area.
- One reach-in refrigerator, one hot holding unit, and one open display case were not in use during inspection.
- Missing a kick plate from the ice machine.
- Ice machine not in use during inspection.
45 Cutting boards from pizza area observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

-Stains observed on the wall in the pizza area.
-Damaged/uneven floor areas observed in the storage room.
-Missing ceiling tiles observed in the kitchen area and in the top of the exhaust hood, ware washing area, and dry storage area.
-Gaps observed in the ceiling in the pizza area.
53 The following observations were noted:
- Grease and food debris observed on the floor in the cooking area.
- Food debris observed on the floor in the sushi area.
- Pool of water observed on the floor in the coolers area (storage) and in the butcher area.
- Floor drains are in need of cleaning in the prep area/ ware washing area.
54 Dust accumulation observed on ceiling vent covers across from the pizza prep area and kitchen prep area.

PIC took corrective action and provided two new light during inspection.

1 The facility's food safety certified handler lacks the City of Philadelphia food safety certificate. Observe a ServSafe certificate only. Advised facility's food safety certified handler to apply for the City of Philadelphia food safety certificate. *Application form issue today 6/8/2022.
8 No sign or poster posted at the hand wash sink in the butcher area to remind food employees to wash their hands.
**Sanitarian advised the person in charge to provide the sign for the hand wash sink during inspection.
13 Dirty dishes and equipment observed in the prep sink from the prep area.
PIC removed the dirty dishes and the equipment from the prep sink during inspection.
14 The following observations were made:
-Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours in the lunch meat department.
-Pink slime observed on the interior of the ice machine bin.
-The can openers had an accumulation of food residue on the food contact surfaces in the prep area and pizza area.
**Sanitarian advice the person in charge to removed the ice and wash, rinse and sanitized the ice bin, the slicer, and the can openers.
-The ware washing (3-compartiment) sink not properly setup for sanitation, observed dirty pots and pans inside in all 3 compartments. Corrective action: Sanitarian educated the PIC on how to properly setup the 3-compartment sink for sanitation of utensils and food equipment. A return demonstration of Wash-Rinse-Sanitize setup was observed prior to completion of inspection.
21 Opened commercially processed ready to eat food (lunch meat), located in the under-counter refrigerator / deli area, and held more than 48 hours, were not marked with the date it was opened.
36 - Visible physical evidence of rodent (mouse droppings) observed on the floor in the ware washing and storage area.
-House flies observed in the pizza area.
37 - Food observed not stored at least 6 inches above the floor in the walk-in freezers, customer area (beverages), and storage area.
- Multiple food items observed uncovered in walk-in cooler from the kitchen area.
PIC took corrective action and covered the food from the walk-in cooler during inspection.
41 Common bowls or spoons with handles are stored in direct contact with bulk food ingredients in the cooking area.
45 The following observations were noted:
-Aluminum foil observed on counter/shelves surfaces in the front food prep area /sushi area and back splash of kitchen stoves, where food contact surfaces are not smooth, easily cleanable and resistant to pitting, cracking, or scratching.
-Cardboard lining shelves observed in the walk-in cooler with vegetables which is not easily cleanable.
-Raw wooden pallets observed elevating food items which are not smooth, easily cleanable and not resistant to peeling and chipping.
-Plastic crates being used for elevation in the walk-in coolers, walk-in freezers, storage area, and retail area.
45 Cutting boards from butcher area observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 The following observations were noted:
-Ice accumulation observed on floor and ceiling surfaces in the walk-in freezer.
-Ice accumulation inside chest freezer in the retail area.
-Pooled water observed on floor surfaces in the meat walk-in coolers and storage area.
-Food debris observed in the open display cases in the retail area.
51 Restroom lacks a receptacle with lid or cover for sanitary napkins in the female restroom.
53 The following observations were noted:
-Damaged/uneven floor areas observed in the storage room.
-Missing ceiling tiles observed in the ware washing area and in the top of the exhaust hood.
53 -Cleaning tools observed on floor surfaces and not elevated or hung on a utility rack in the facility.
-Dead cockroaches on glue trap observed in the pizza area.
