4 An open employee's beverage container was observed in a food preparation area. Corrective action: Coffee removed from the food prep area.
42 Preset tableware is not covered, wrapped inverted or otherwise protected from contamination.
45 Bare wood shelf observed in the warewashing area. Note: Surfaces should be smooth, non-absorbent and easy to clean.
45 -Garbage grinder observed installed on a handwashing sink with an open drain. Note: The garbage grinders may only be connected to a direct drainage connection without gaps. -Cold prep unit ambient temperature is 40 degrees F. Temperature of food moved from the walk-in cooler to the unit is increasing. The unit is not cold enough to hold food at 41 degrees F or below. All potentially hazardous food was moved back to the walk-in cooler. -(Ref. 46.501) The following equipment is not NSF/ANSI approved: Kitchen Aid stand mixer, ice machine. Provide cut sheets for ice machine.
47 Plastic drying racks observed soiled. Clean or discontinue use.
49 -The ice machine does not have a backflow prevention device. -Main backflow prevention device is in disrepair. Leak observed.

11 Visible evidence of insect contaminated foods in the basement storage area. Insects were observed in the bulk containers of flour and rice. Instructed the person in charge to discard items.
13 -Garbage grinder observed on the food preparation sink. -Condensation hose for the walk-in cooler was observed draining into the sanitizing solution in the warewashing sinks. Hose removed. COS
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Corrective action: The concentration was corrected during the inspection.
14 Clean dishes observed stored in soiled drying rack.
14 Cell phone observed on cutting board at cold prep station. Corrective action: Cell phone removed and surface sanitized.

1 Food employee was observed sanitizing dishes prior too rinsing them. It is the duty of the person in charge to routinely monitor sanitizing procedures to ensure that they are done correctly. Corrective action: When incorrect procedure was brought to the attention of the person in charge, he showed the employee the correct warewashing method. (wash, rinse, sanitize, air-dry)
11 Drink ice observed stored in a reusable plastic grocery bag. Corrective action: Ice was discarded. Note: Food like ice must be stored in food-grade bags or food-grade containers.
36 Rear outer opening in the food facility do not protect against the entry of insects and other animals. Gap at the bottom of the exterior door.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Food safety certified person's cert expired. Corrective action: Another food safety certified person arrived with a valid certificate.
2 Food safety certified person was not able to demonstrate knowledge of reportable foodborne illnesses and symptoms. Certified person with expired cert. was able to demonstrate knowledge.
7 A food employee was observed touching ready to eat food with his bare hands (lettuce). Please use tongs or wear gloves when touching ready to eat food. Instructed the person in charge to discard the lettuce and to get a new plate.
8 Paper towel dispenser empty at the handwash sink in the wait-staff area, prep area, customer toilet room and employee toilet room. Corrective action: Instructed the person in charge (PIC) to put napkins at all hand-sinks. PIC complied.
8 -Per the person in charge, the hand-washing sink is being used to wash food. Hand-washing aids were observed at the sink and a hand-washing sign is posted. No food prep sink was observed. -Pan observed in the handwashing sink.
13 Uncovered food was observed in the walk-in cooler. Instructed the person in charge to cover the food.
13 Water was observed dripping from the fan housing in the walk-in cooler onto the raw produce. Instructed the person in charge to move food from under the leak.
