45 The following was observed:
-Non Nsf approved refrigeration units in (Older Toddler, Infant, Young toddler) rooms.
-Kitchen had 1 Non NSF approved refrigerator and a Non NSF approved microwave
-2 Non NSF approved microwaves in the classrooms
-Cooking equipment was not in service in the kitchen (Stove, Hood system)
48 Hot water knob not working in the bathroom of the PHL3 room. They use the handwash sink next to the bathroom.
**Person in charge told the plumber. Corrective action was taken.
53 The following observations were noted:
- Porous ceiling tiles in the kitchen area.
- Gaps observed in the ceiling around the utility pipes in the kitchen area

45 -Domestic refrigeration unit and microwave observed in the kitchen.
53 The following observations were noted:
- Porous ceiling tiles in the kitchen area.
- Gaps observed in the ceiling around the utility pipes in the kitchen area and in the bathrooms (first floor) area.
- Hole observed in the ceiling tiles (near the restrooms) in the hallway.
54 - Cover vents observed with dust accumulation in the hallways (near restroom).

6 The children and teachers hands can not be wash properly in several classrooms and bathrooms in the facility.
The hot water at the hand wash sinks was measured between 62 degree F to 64 degrees F in two classrooms and one bathroom first floor and in 2 classrooms and two bathroom first floor.
*Sanitarian advice the person in charge to use the hand wash sinks for the bathrooms and classrooms where is hot water at least 100 degrees F.
8 The hot water at the hand wash sinks was measured between 62 degree F to 64 degrees F in two classrooms and one bathroom first floor and in 2 classrooms and two bathroom first floor.
14 - Multiuse utensils observed being used in the cans and jars in the pre-k 2 classroom without sanitization.
- Brooms and dust pans observed in top of toys in PHL 3 classroom.
36 Visible physical evidence of insect activity (cockroaches) observed on the floor in the kitchen area.
45 The following observations were noted:
-Domestic refrigeration unit, microwave, toaster oven and toaster observed in the kitchen.
- Two domestic refrigerators and two domestic microwaves observed in the classrooms.
- Damaged faucet hot water side in the PHL 3 bathroom.
47 -The floors on first floor observed with debris and dust accumulation.
-The carpet pre-k classroom observed with stains and debris.
-The convection oven/kitchen area and microwave "Faberware" are in need of cleaning.
49 -No hot water provided in all the classrooms and bathrooms.
53 The following observations were noted:
- Missing ceiling tiles in the kitchen area.
- Porous ceiling tiles in the kitchen area.
- Damaged floor tiles in the kitchen area.
- Dusty window seals in the kitchen area and first floor classrooms.
- Water stains ceiling tiles in the storage are.
53 Brooms and dust pans observed in top of toys in PHL 3 classroom and not elevated on the utility rack.
54 - Cover vents and heater covers observed with dust accumulation throughout the facility.

- Hot water not present in several classrooms and three bathrooms. The water was measured between 64-68 degrees F.
3270.76 - The floors are in need of cleaning in the kitchen area and pre-k classroom.
- Missing ceiling tiles and water stains observed in the kitchen and storage area.
3270.102(a) Brooms and dust pans observed in top of some toys in the closet area.
