1 -The facility's food safety certified handler lacks the City of Philadelphia food safety certificate. Observe a ServSafe certificate only. Advised facility's food safety certified handler to apply for the City of Philadelphia food safety certificate. *Application form issue today 2/28/2025.
8 -Paper towel dispenser empty at the handwash sink in the restroom 2nd floor area.
13 -Observed a plastic container with bread uncovered in the bar area.
-Observed 8 pieces of ground meat unwrapped in the under-counter drawer /cooking area.
-Spoon observed buried inside the ice bin bar area.
-Uncovered ice bin in bar area (ice being use for drinks).
*PIC discarded 8 pieces of ground meat during inspection.
21 No dates observed on several food items (cheese, ground meat, shrimp, salads) inside the walk-in cooler/kitchen area.
36 - Visible physical evidence of rodent (mouse droppings) activity observed on the floor in the kitchen and bar area.
37 Food is not stored at least 6 inches above the floor inside the walk-in cooler first floor.
41 Food dispensing utensil (dirty knife) in under-counter drawer observed in direct contact with food (ground meat) and not having handle stored above the top of the food.
PIC removed the knife during inspection.
43 -Plates are not stored inverted in the bar area.
45 Plastic crated being used for food elevation throughout the facility.
45 - Several cutting boards observed with deep scratches not allowing for effective cleaning and sanitizing in the kitchen area.
47 Debris observed inside the under-counter freezer kitchen area.

51 Missing toilet tissue in the woman's restroom 2nd floor restroom.

-Missing base coving observed in the second floor storage area.
-Damaged floor observed in the cooking area.
-Damaged walls observed in the second floor storage and bar first floor (near the end).

-Pool of water observed on the floor 2nd floor in front of the walk-in freezer.
53 Maintenane tools observed on the floor in the facility.

1 The food safety certified handler was not present at the start of inspection, he arrived prior to completion of inspection.
13 -Observed a plastic container with food in top of uncovered container with marinated bacon.
-Spoon observed buried inside chicken salad container /walk-in cooler.
*Sanitarian advised the person in charge to discarded the marinated bacon.
14 - Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
-The can opener had an accumulation of food residue on the food contact surfaces.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available inside the walk-in cooler and several chest refrigerator units.
35 - Several working bulk food ingredient storage containers are not labeled with the common name of the food in the storage area second floor.
36 - Observed two insect control devices (fly zappers) and two sticky fly traps located in the kitchen area with potential to contaminate food, equipment, and / or utensils.
*Sanitarian instructed the person in charge to removed and/or disconnect fly devices form the kitchen area.
37 Food is not stored at least 6 inches above the floor inside the walk-in cooler.
37 Uncovered containers with food observed inside the under-counter freezer kitchen area.
PIC covered the food during inspection.
