1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. ***Staff members have ServSafe certificates Corrected on Site: An email with the food safety certificate application will be sent.
4 Food employee observed eating food in the front food prep area.
6 Observed three food employees donning single use gloves without a prior hand wash.
8 Observed pot inside of handwashing sink in food prep area. Corrected on Site: Was removed upon request.
13 Observed uncovered chicken, beef, and potato balls inside of walk-in refrigerator. Corrected on Site: Items were covered upon request.
13 Observed dirty dishes inside of prep sink.
18 Observed the following: - Stews and soups not cooled from 135 degrees F to 70 degrees F in 2 hours and/or from 135 degrees F to 41 degrees F within 6 hours, after preparation. Temperature at the time of inspection was 85F after cooling down for almost 2 hours. - Cooked chicken cooling down on food prep table measuring 95.7F Corrected on Site: Stews and soups were reheated and chicken was placed in refrigerator during inspection.
20 Cooked pork chops was held at 52 degrees F rather than 41 degrees F or below as required. **Pork chops were sitting out waiting to be reheated at time of inspection. Corrected on Site: Pork chops were placed in walk-in cooler upon request.
26 Observed unlabeled chemical bottle in front food prep area.
36 Visible physical evidence of insect activity (One roach and flies) observed in the rear kitchen area.
36 An insect control device (sticky fly trap) located in rear food prep area with potential to contaminate food, equipment, and / or utensils.
37 Food is not stored at least 6 inches above the floor inside of walk-in cooler.
41 Observed the following: - Knives and / or cleavers stored between the three compartment and the wall which were not easily cleaned and sanitized. - Common bowl without handles inside of bulk salt **Knives were washed, rinsed, and sanitized, bowl was not removed.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
47 Observed the following: - Grease accumulation on exhaust hoods in front and rear cooking areas. - Dust accumulation on fan inside of walk-in cooler. - Microwave in rear food prep area that has an accumulation of food debris
51 Observed dust accumulation on vents in both restrooms.
53 Broom and dustpan were not properly elevated in the facility at time of inspection.

8 No sign or poster posted at the handwash sink in the kitchen area to remind food employees to wash their hands. The person in charge was instructed to take corrective action. Person in charge put a sign at the handwash sink at time of inspection
34 Temperature measuring devices for ensuring proper temperatures of food were not available or readily accessible.
35 Numerous containers of food were not properly labeled in the walk-in cooler.
37 Beverages were not stored at least 6 inches above the floor in the basement.
39 Moist wiping cloths observed lying on prep table and not stored in sanitizing solution.
