8 The following was observed:
- Soap was not available at the handwash sink in the bathroom.
- Paper towel dispenser empty at the handwash sink in the bathroom.
**Person in charge took corrective action.
34 Food temperature measuring devices for ensuring proper food temperatures are not calibrated and/or functioning properly.
37 Food is not stored at least 6 inches above the floor.
45 The following were observed:
-Broken door handle on walk-in cooler
-3 bay ware-washing/garbage grinder sink being used for dry storage
-Not in use slushy machines (2)
47 The following were observed:
-Ice accumulation on interior surfaces of chest freezer
-Static dust and debris on lower interior surfaces of reach-in refrigeration units/open display case unit
48 Hot water was not turned on.
**Person in charge turned the hot water on during the inspection to 100 degrees F.
51 Static dust on air vent in unisex employee restroom
54 The following was observed:
- 2 lights out in the retail area
- 1 light out in the reach in unit

13 An employee stored an open canned soda inside the chest freezer where the unopened ice cream was located.
**Person in charge removed the soda.
20 Open display case food was held at 65 degrees F rather than 41 degrees F or below as required.
**Person in charge removed everything from the open display and placed in the walk-in cooler.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in reach-in refrigeration units/open display case.
45 The following were observed:
-Milk crates being used for elevation of food in retail area, which are not smooth and easily cleanable
-Carboard being used to line lower surfaces of shelving units in reach-in refrigeration unit, which are not smooth and easily cleanable
-Raw wood being used to support handwashing sink in unisex employee restroom, which is not smooth and easily cleanable
45 The following were observed:
-Broken door handle on walk-in cooler

-Open display case not showing 40 degrees F or below.
-Not in use slushy machine
47 The following were observed:

53 Static dust and debris observed around perimeters of floor in retail area.

**PIC took corrective action and increased hot water heater for hot water to reach peak demand in one to two minutes.
51 The following were observed:
-Static dust on air vent in unisex employee restroom
-Residue around exterior surfaces of toilet in unisex employee restroom
53 Static dust and debris observed around perimeters of floor in retail area.
