45 Defective reach-in freezer and chest freezer observed.
47 - Food debris observed on bottom surfaces of reach -in freezer. -Dust accumulation observed on kick plate of reach-in unit in prep area.
51 -Dust accumulation observed on ceiling vent in restroom.
53 - Old mouse droppings observed on floor surface corner under chemical storage rack. - Debris and dust observed on floor surface under equipment. * General cleaning needed.

45 The following observations were made: -Non-NSF/Ansi certified reach-in freezer. - Kick plate observed dusty on reach-in refrigerator. -Fan vent cover missing off of vent in reach-in refrigerator. - Domestic microwave observed. - Employee use only sign is needed. -Handle observed loose on reach-in freezer.
47 - Food debris observed on bottom surfaces of reach -in units in the facility. - Dust accumulation observed on fan vent covers in reach-in units. -Dust accumulation observed on kick plate of reach-in unit in prep area.
51 - Faucet observed loose at hand wash sink in 1st restroom. -Dust accumulation observed on ceiling vent in restroom.
53 Mop observed in mop bucket and not hung to allow for proper air drying.
