14 There was no detergent readily available in the bar warewash machine. There was a chlorine sanitizer, a rinse aid bottle and a quarternary ammonium sanitizer bottle in this unit. The person in charge changed out the quarternary ammonium bottle for the detergent bottle.
14 A bar 3 basin sink was set up to wash and sanitize only. The inspector requested that the sink be set up properly to wash, rinse and sanitize. An employee drained and set up the sink properly.
20 Hamburgers, pork, and shrimp was held at temperatures above 41 degrees F rather than 41 degrees F or below as required. Since these items were stored in this unit for four hours or more they were discarded.
38 Food employees observed not wearing or wearing ineffective hair restraints (beard restraints).
45 Splashguard lacking between the garbage grinder sink and the food prep sink. Note: The person in charge stated that the prep sink is not used. All produce ingredients come prepackaged.
45 -The garbage grinder sink in the kitchen was lacking water. The water was turned on at the valve below. -A bar refrigerator is making a loud rattling sound. The ambiant air was scanned at 52 degrees F. The unit temperature display read a lower temperature. Only prepackaged beverages were stored in this unit. -The bar handsink sign is faded.
47 -Residue on floor perimeters inside the walk-in unit.
53 -Debris observed on floor perimeters of the bar and kitchen areas. -Residue observed on the wall surfaces in the kitchen area.
54 A designated place for employees to store personal items is lacking. Employee belongings observed stored on top of boxes and in grinder sink in the bar and kitchen area.
63 Person in Charge of premises has not posted the appropriate warning signs regarding the dangers of consuming alcoholic beverages during pregnancy.

14 The 3 basin sink was set up to rinse, wash, and sanitize rather than wash, rinse, and sanitize. The person in charge drained the basins and set up the sink properly.
36 Visible physical evidence of insect activity (small flies) observed in the bar area.
45 -The soap dispenser is not attached to the wall in the kitchen. -The bar handsink sign is faded.
45 Splashguard lacking between the garbage grinder sink and the food prep sink.
46 Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration.
46 Brown residue observed on the bar dishwashing machine inner surfaces.
47 -Grease and soil accumulations observed on cooking equipment surfaces. -Residue inside refrigeration units and walk-in units in the kitchen, bar, and rear storage areas.
53 Direct floor storage of cleaning equipment(broom and mop) observed stored on the floor rather than hanging on a utilty rack in the kitchen. Maintenance tools are not properly elevated to prevent vermin attraction.
53 -Dust and debris observed on floor perimeters of the bar and kitchen. -Residue observed on the wall surfaces in the kitchen area. -Unused box refrigerator in the rear bar area.
54 One of three lights is not working in the exhaust hood in the kitchen.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The person in charge has an expired food manager food safety certificate. The person that arrived does not have a Philadelphia Food Manager Food Safety Certificate. The application will be emailed with this report.
8 The hand sinks throughout began lacking water at 100 degrees F during the inspection. The maximum temperature probed was 82 degrees F in the kitchen. Hot water at or above 100 degrees F was provided while the report was typed. Hot water at 100 degrees F was provided while the report was prepared.
14 Bar gun contained red and brown residue on the beverage contact surfaces. The person in charge cleaned and sanitized it during the inspection.
21 -Prepared fruit in the bar area in jars were lacking dates. -Opened and prepared foods (cheese) were lacking use by or made on dates in the kitchen refrigeration units. Dates were placed on these items.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
45 -Rust on bar beverage box refrigeration unit inner surfaces. -The bar dish wash machine is not working. -A kitchen refrigerator light is not properly attached or working.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 -Grease and soil accumulations observed on cooking equipment surfaces. -Residue inside refrigeration units and walk-in units in the kitchen, bar, and rear storage areas.
48 There was no water readily available at 100 degrees F in the kitchen. The maximum temperature probed was 82 degrees F. Hot water at 100 degrees F was provided while the report was prepared.
