1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Person In Charge has an expired city certificate. The application to apply for or renew the Food Safety Certificate (FSC) has been emailed to the PIC.
1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
**Person in Charge was not present in the facility at the beginning of the inspection, but arrived later during the inspection.
8 -Soap was not available at the handwashing sink in the rear ware washing room.
21 Opened commercially processed ready-to-eat food (cheese and lunch meat) in the salad prep unit near the cash register counter was not date-marked.

**PIC made adjustments to the hot holding unit during the inspection, and the food temperature was later measured at 146 degrees F.
20 Cheese was held at 61 degrees F rather than 45 degrees F or below as
required in the salad prep unit near the pizza counter. Potentially hazardous food (chopped lettuce, sliced tomatoes, etc.) was held at 66.7 degrees F instead of 41 degrees F or below, as required, in the salad prep unit near the pizza counter.
**PIC moved above food items to the walk-in cooler.
