9 Food facility Person in Charge not able to provide documentation that pretzels foods are from an approved source. Pretzels was removed during the inspection.
13 Uncovered cheese and deli-meat observed inside the closed display case refrigerator in the prep area. Person in charge rewrapped deli meat during the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 150 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. Additional pills added to solution to derive 200 ppm during the inspection.
39 Moist wiping cloths observed lying on prep sink and not stored in sanitizing solution.
45 -Splash guard needed in the prep area between the prep sink and the handwash grinder sink. -Domestic microwave observed in the prep area during the inspection.
45 Warewash sink, prep sink and Hand wash sink with grinder disposable machine was observed in the prep area, not properly sealed to the wall.
47 Ice accumulation observed inside chest freezer in the front retail area.
53 -Cleaning needed on floor corners and perimeters of prep, retail and basement storage areas to remove debris and grimes observed during the inspection. -Acoustic ceiling tiles observed in the prep area and throughout the store. -Water stained ceiling tiles observed in the rear area of the establishment.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Servsafe observed and city certificate application emailed to manager during the inspection.
8 Soap was not available at the handwash sink in the restroom area. Soap was replenished during the inspection.
13 Uncovered ready to eat food items observed inside closed deli case during the inspection. Person in charge rewrapped all food items such as cheese and deli meat during the inspection.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 50 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. pills added to solution during the inspection to derive 400 ppm.
21 Deli meat and cheese opened commercially processed ready to eat food, located in the closed display case, and held more than 48 hours, was not marked with the date it was opened. Person in charge of the prep area was able to remember and put the dates on each of the opened food items and discarded the ones dates of opening could not be recalled.

53 -Dirt observed in mop sink. -Seal holes and gaps in walls near basement trap door. -Inoperable freezer chest used to store soft drinks. -Stripping paint and water damaged walls in toilet room. -Water stained ceiling tiles in prep area.
53 -Fissure acoustic ceiling tiles, which are porous, not smooth and easily cleanable observed in food prep area. -Seal around penetrating plumbing and electrical lines. -Large gap between wall and floor in toilet room.
