8 Paper towel dispenser empty at the handwash sink in the toilet room area.
They were provided.
13 Produce (tomatoes and lettuce) are served on the deli sandwiches. There is no indirectly drained food prep sink.
The person in charge states that he purchases prepackaged lettuce.
Discontinue the washing of tomatoes at the 3 basin sink in a plastic bowl.
14 Slicer has an accumulation of dried food residue on the inner food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. The small parts should be placed in the 3 basin sink for cleaning and the parts that can not be moved should be cleaned in place.
The person in charge began cleaning the slicer during the inspection.
20 Prepackaged prepared salads were held at 54.8 degrees F rather than 41 degrees F or below as required in the walk in cooler.
The 3 containers were discarded.
Do not use the walk in cooler to store potentially hazardous foods until it can keep them at the required temperature.
21 Deli meats and cheeses, opened commercially processed ready to eat food, located in the deli display case, and held more than 48 hours, was not marked with the date it was opened. The date stickers on the windows were from July and August.
-Instructor requested that dates be provided for the open foods in the closed display case.
26 -Observed rodenticides (open and uncontained poison packets) not labeled by the manufacturer for food facility usage, in the retail area.
-Observed insecticides (Hot Shot and Black Jack insect sprays) not labeled by the manufacturer for food facility usage, in the prep/warewash area.
The inspector requested the 4 cans of spray be discarded.
Note: All pest control should be done by a licensed exterminator.
26 -Chemicals stored next to and over food items in the warewash/prep area.
The foods were moved away from the chemicals.
35 -Prepackaged (peanuts) food is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 -Visible physical evidence of insect (live large roaches and flies) activity observed in the warewash/prep and rear storage areas.
43 Stirrers in the self service coffee area are not prewrapped.
45 -Plastic crates used to elevate items rather than NSF approved dunnage racks.
-Damaged foil observed lining shelves and the wall in the warewash/prep area rather than a covering that is resistant to damage, smooth and easy to clean.
45 Observations:
-The walk in cooler is not keeping foods at 41 degrees F or lower.
-Damaged glass door is taped on the reach in freezer in the retail area.
-Large refrigerator is not working with doors removed used for other storage in the office area.

-Large dead roaches and debris observed on a shelf in the warewash/prep area.
-Dust, accumulated food debris and residue observed in the refrigeration units throughout.
-Static dust observed on surfaces (around the condenser fans, ceiling and walls) in the walk in refrigerator.

-The sink is not sealed to the wall.

-Live large roach observed crawling on the wall in the rear storage area.
-Excess boxes and other items stored in the office and rear storage areas.
-Unused meat cutter observed in the deli prep area.

The cans were removed from the retail shelf to be returned to distributor or discarded.

14 Slicer has an accumulation of dried food residue on the food contact surfaces (around the blade guard and other surfaces) and is not being washed, rinsed and sanitized and air dried at least every 4 hours.
An employee began washing, rinsing, sanitizing and air drying the deli slicer.
21 Deli meats and cheeses, opened commercially processed ready to eat food, located in the deli display case, and held more than 48 hours, was not marked with the date it was opened.
-Instructor requested that dates be placed on a log for the open deli items.

26 -Employee personal hygiene products (listerine) stored in warewash area, and were not stored to prevent the contamination of food, equipment or utensils.
-Personal hygiene products (perms, shampoos, lotions, and other items), glue and hand sanitizer stored in cashier area, and were not stored to prevent the contamination of food, equipment or utensils.
-Chemicals stored over beverages in the retail area.
The chemicals were moved away from the beverages.
35 -Prepackaged (peanuts and crackers) food is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
-Prepackaged turkey tails are lacking the distributor name.
36 -Visible physical evidence of rodent (mouse droppings)/ insect (flies) activity observed in the office, toilet room and prep areas.
-Full glue traps containing large dead insects observed.

-The electric griddle is not commercial or approved. There is no commercial exhaust hood.
-Large refrigerator is not working with doors removed used for other storage in the office area.

-The meat cutter had accumulated food debris and residue in the upper cabinet. The meat cutter was stored against the wall so the bottom cabinet was not accessible.

-Accumulated food debris and residue observed in the refrigeration units throughout.
-Accumulated static dust observed on surfaces (around the condenser fans, ceiling and walls) in the walk in refrigerator.
-Accumulated static dust on shelving in the warewash area.

-Mouse droppings, full glue traps, open and uncontained poison packets observed on floor perimeters. For example, office area under the shelves and in corners, warewash area, near the meat cutter and under shelves and near the outer wall in the retail area.
-Accumulated static dust has accumulated on surfaces near the compressor unit for the deli refrigerator.
-Excess boxes stacked in the office and rear storage areas.
53 -Acoustic, water stained and damaged ceiling tiles observed throughout.
-Openings in surfaces observed in storage and office areas.
53 -Brooms and dust pans are stored on the floor rather than hanging.
-Mop observed stored in the bucket with soiled water rather than hanging.
54 An unnapproved non-commercial electric griddle is in use in the ware washing room without a mechanical exhaust hood to safely vent grease and heat.
54 Employee's belongings stored on the meat cutter rather than in a designated area away from prep surfaces.
56 Person in Control of premises has not posted "No Smoking" signs.
