6 Observed the handwashing sink in the women's restroom without any hot water. **The women's restroom was placed out of order at the time; gender neutral restroom will be used until hot water has been resupplied to the sink.
8 The handwashing sink in the gender neutral restroom did not have single use towels, continuous towels, or air drying device. **Paper towels were supplied at the time of the reinspection.
13 Observed the handle of the ice scooper in contact with the ice in the bin at the bar area. **The ice scooper was removed from the ice bin at the time of the reinspection.
37 Observed a spill on the bottom shelf of the reach-in refrigerator from a damaged container storing chicken.
45 Observed storage container containing chicken in the reach-in refrigerator within the kitchen area cracked/damaged.
48 Observed the women's restroom lacking hot water.

26 Observed insecticide (Chemical releasing fly trap) and rodenticide (loose mouse bait) not labeled by the manufacturer for food facility usage, in the basement, bar and women's restroom area. **All insecticide traps in unapproved areas were discarded at the time of the inspection.
36 Visible physical evidence of rodent activity (droppings & dead rodent body) observed in the kitchen & basement area.
45 Observed mini freezer behind the bar area with an accumulation of build up ice.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
49 Observed the handwashing sink in the kitchen area clogged/slow draining. **The person in charge unclogged the handwashing sink at the time of the inspection.
53 - Observed debris on the floor underneath the cooking equipment and refrigerator unit in the kitchen area. - Observed mouse/rat droppings underneath the 3-bay warewash sink in the kitchen area. - Observed fallen containers and debris in the kickplate of the chest refrigerator at the bar area. **Debris observed in the kickplate behind the bar area was cleaned up at the time of the inspection.
