1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Food safety certified person was not present at the Sanitarian's arrival. Food safety certified person arrived at the end of the inspection.
8 Paper towel dispenser empty at the handwash sink in the basement prep area. PIC provided paper towel at the time of the inspection.
26 Observed insecticides (Real Kill Ant & Roach) not labeled by the manufacturer for food facility usage, in the prep area. PIC discarded at the time of the inspection.
41 Common bowl or food container (flour) observed without handles in direct contact with bulk food ingredients.
45 Non-ANSI/NSF sanitation approved microwave oven observed in the prep area.

51 Dust accumulation observed on the vent cover in the restroom.
53 The following observations were made: - Dust accumulation on wall vent toward rear exit door. - Porous ceiling tiles observed in the basement prep area. - Unsealed gaps around utility lines in the basement.
53 Brooms/dustpan were not elevated when not in use.
