6 Food Employee did not follow proper handwashing procedure. Soap and/or warm water not used.
No hot water was available in the men toilet room. Water temperature measured between 57-64 degrees F instead of minimum 100 degrees as required.
-No soap available at the handwash sink in the women's toilet room.
8 The following observations were noted:
-No hot water at handwash sink in the men toilet room. Water temperature measured at between 57-64 degrees F instead of minimum 100 degrees as required.
-Soap dispenser not working as designed, no soap available at the handwash sink in the women's toilet room.
37 The following observations were noted:
-Bottom bread crates used as storage not elevated at least 6 inches off the ground in the storage space near loading dock.
-Food employee storing food directly on floor surface and restacking food items on top of each other in the retail area.
45 The following observation were noted:
-Damaged and undamaged raw wooden pallets used for elevation of food throughout the facility. (Walk-in coolers, retail and storage areas)
-Cardboard boxes used as display storage throughout retail rea.

11 Observed 11 severely dented, swollen, distressed canned items on display for purchase on retail shelves throughout retail area.
37 The following observations were noted:
-Bottles and containers filled food items left on direct floor surfaces in the walk-in cooler and in the storage space near loading dock.
-Bottom bread crates used as storage not elevated at least 6 inches off the ground in the storage space near loading dock.
-Bottom storage racks located in the walk-in freezer used to store food is not at least 6 inches off the ground.
45 The following observation were noted:

-Grocery shopping cart used for storage in the walk-in freezer.

-Debris and spillage on floor surfaces in the walk-in cooler/freezer.
-Bottom internal surface of reach-in refrigerator located in storage area not clean to sight.
-Heavy ice accumulation on internal surface of reach-in freezer located in the storage area.

-Debris, trash and wood chips on floor surfaces in the throughout storage area.
-Debris on shelves in the "Chips" section of the retail area.
-Debris on top external surface of reach-in freezer in the retail area.

-Idle mop stored on direct floor surface of mop sink, not elevated on utility rack.
-Idle mop stored in mop bucket with stagnant cloudy water near loading dock area.
