8 Paper towel dispenser empty at the handwash sink in the employee toilet room.
12 Shellstock tags are not maintained for 90 days from the date the container is emptied.
13 Food items (chicken, rice and sauce) observed uncovered in the walk-in cooler. PIC covered food items at the time of the inspection.
14 Black residue, pink slime observed on the interior of the ice machine bin.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. Food employee wash, rinse and sanitized the slice at the time of the inspection.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher in the bar area was 10 ppm, and not 50-100 ppm as required. PIC primed the dish machine twice. Proper concentration is now observed.
21 - Potentially hazardous ready to eat foods (rice and noodles), prepared in the food facility , located in the walk-in cooler, was not date marked. PIC date marked the food items at the time of the inspection.

13 - Several food items observed uncovered in the walk-in cooler.

20 -Defective under-counter refrigeration unit stored food (crab cakes) at 63 degrees F in the cooking area.
*Sanitarian instructed the person in charge to discarded above mentioned food items immediately and discontinue usage of the under-counter refrigerator unit until is repair.
21 - Several food items observed without the dates in the walk-in cooler.
*Sanitarian instructed the person in charge to provide dates for all the food in the walk-in cooler.
**Refrigerated ready-to-eat potentially hazardous food prepared and packaged by a food processing plant shall be clearly marked with the date the original container is opened in a food facility.
31 The under-counter refrigerator with crab cakes was observed at 63 degrees F instead of 41 degrees F or less in the kitchen area.
PIC relocate the food from under-counter refrigerator during inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available in the facility.
36 Visible physical evidence of rodent (mouse droppings) observed in the ware washing area on the first shelf with cutting boards and insect activity (house flies) observed in the cooking area.
36 Fly glue strip observed in the kitchen food prep area with potential to contaminate food equipment and utensils.
PIC took corrective action and removed the fly glue strip during inspection.

-Cracked floor tiles observed underneath the dishwasher sink in the bar area and kitchen area.

PIC discarded the chicken during inspection.

-Missing bottom face plate observed on the cold prep unit in the food prep area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 - Shelving in the bar area observed with dust accumulation.

-Grease accumulation observed on the lower outer surfaces of the fryer and food equipment in the food prep area.
-Rust observed on floor/wall and ceiling surfaces in the walk-in cooler (in the kitchen area).
