2 The food safety certified person (FSC) could not demonstrate adequate knowledge of the foodborne illnesses deemed reportable and the symptoms that would require employee exclusion. Sanitarian reviewed the foodborne illnesses with the FSC person during the inspection and emailed a sample health policy with this report.

36 Visible physical evidence of rodent / insect activity observed in the food prep/warewashing area. Mouse feces was observed in the following places: -Counter behind the cookie display & condiments. -Floor beneath the warewashings sinks. -Floor in basement storage area.

43 Single use cardboard boxes being reused for further food storage. Discontinue using previously used boxes for pretzels.

43 -Boxes of food containers were observed stored beneath a sewer pipe that is not shielded to intercept drips. -Spoons near the register are not stored in a manner in which contamination of food- and lip-contact surfaces is prevented.

45 -Rusting, peeling racks observed in the walk-in cooler. -Crates used to elevate equipment and storage. -Exterior thermometer observed broken on the walk-in cooler.

45 Raw wood rack used for storage in basement. Note: Surfaces should be non-absorbent, smooth and easy to clean.

47 -Ice accumulation observed in chest freezers. -Debris observed on rim of chest freezer with sliding glass doors. -Mouse feces on counter behind cookie display was cleaned during the inspection.

53 -Cleaning is need to eliminate mouse droppings on the floor where needed. -Remove any items that are not necessary for the operation of the business. -Debris observed on basement floor.

54 Light in the walk-in cooler should be at least 110 lux.
