8 The handwash sink in the food prep area was blocked by a trash can. Sink must be easily accessible for employee use. Corrective action: Trash can moved.
13 Milk that was out of temperature was accessible to customers in the retail area while waiting for return to distributor. Corrective action: Milk was moved to the lower compartment of the deli case for return to the distributor.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Food employee attempted to correct the concentration, but did not. Sanitarian instructed the person in charge and the food employee to determine how much water is being placed in the sink so that the proper amount of tablets can be added to the water.
20 -Potentially hazardous food was held at 44.1 to 44.8 degrees F rather than 41 degrees F or below as required. Corrective action: The deli case temperature was lowered and started to go down during the inspection. -Milk held at 61.7 degrees F, rather than 45 degrees F or below as required. Corrective action: Milk was removed from the retail area and placed in the lower compartment of the deli case for return to the distributor.
26 Loose rodent bait observed on basement floor. Corrective action: All loose bait was removed from the floor during the inspection.
36 Outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. -Grate with holes observed in Bilco door in basement. -Open window without screen in basement.
41 Food dispensing utensil in the seafood salad observed in direct contact with food and not having handle stored above the top of the food. Corrective action: Handle removed.

14 -The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 50-100 ppm as required. -The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Corrective action: Sanitizer solution was prepared with only quaternary ammonia tablets during the inspection instead of bleach and quat.
21 -Opened commercially processed ready to eat food, located in the deli case, was not marked with the date it was opened. (deli meats, cheeses, protein/meat salads) -Potentially hazardous ready to eat food (cut watermelon), prepared in the food facility, located in the reach-in refrigerator, was not date marked.
26 -Bleach not labeled for use on food contact surfaces was observed used for sanitizing dishes. -Manufacture's use instructions for quaternary ammonia tablets were not followed. Bleached was mixed in with the quaternary tablets. Corrective action: The water was let out and new sanitizer was prepared.
35 Cereals in clear plastic bags are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
37 A splashguard is needed between the handwashing sink and the cold prep refrigerator/sandwich station.
38 Food employees observed not wearing hair restraints, such as nets, hats, or beard restraints.
