13 Raw eggs observed above other raw, ready-to-eat food items in the walk- in cooler. *PIC relocated eggs at the time of inspection*
36 Large hole observed in the wall behind the office entry door (connecting to wall of kitchen) with physical rodent evidence inside (mouse droppings). This outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
36 Visible physical evidence of rodent activity observed:
-mouse droppings observed on lower shelving in the kitchen prep area
-mouse droppings observed along floor perimeters in the office/ dry storage
-mouse droppings observed along floor perimeters in the bar area and on lower shelving in the bar area
37 multiple food items observed not stored at least 6 inches above the floor: potatoes, onions on the floor in the prep area; containers of prepped food on the floor in the walk-in cooler.
43 Single-service / single-use cleaned and sanitized utensils shall be handled, displayed and dispensed so that contamination of food- and lip-contact surfaces is prevented. *PIC inverted utensils at the time of inspection*

-blown out lighting (2 of 4) observed in the kitchen grill top area
45 Cutting boards attached to the salad prep units observed with deep scratches and scoring, not allowing for effective cleaning and sanitizing.
45 Observed dining cloth napkins/ towels being used to line shelving in the prep area- these towels are easily cleanable; however, they are not replaced often and have heavy food accumulation observed on them.

-Ice build-up observed on the interior of the mini reach-in freezer at bar

8 No sign or poster posted at the handwashing sink in the bar area to remind food employees to wash their hands.
**Person in Charge placed a temporary sign at the handwashing sink during the inspection.
13 Food items (dumpling shaped items) observed uncovered in the salad prep unit and in the reach-in refrigerator unit.
**Person in Charge provided cover to food items during the inspection.
14 Pink slime observed on the interior of the ice machine bin in direct contact with ice.
**Person in Charge removed ice, cleaned and sanitized the ice machine bin during the inspection.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required.
**The sanitarian instructed the facility to utilize the 3-bay ware washing sink until the dish machine is serviced or repaired.
36 A restricted fly zapper observed placed on the food storage unit in the rear storage area.
**Person in charge removed the fly zapper during the inspection.
37 -Cases of beer observed stored on the floor near the office room.
-Boxes of potatoes observed stored on the floor under the food prep table in the kitchen.
-Food ingredient containers observed stored on the floor in the rear storage area behind the refrigeration units.

45 -Equipment does not meet NSF/ANSI sanitation standards: "Whynter" reach-in refrigerator in the bar area.
-The single door reach-in refrigerator lacks a condensing unit cover/kickplate.

-Dusty vent covers observed on the exterior of the ice machine.
-Food residue observed on the interior surfaces of the salad prep units.
-Dusty fan guards observed in the walk-in coolers.
52 Observed stacks of cardboards on the floor blocking the entrance to the trash room.
53 -Observed stained ceiling and wall surfaces throughout the kitchen area.
-Food debris and grease build-up observed along the floor/wall junctures of the kitchen area.
54 Observed one light was out under the hood.
54 Lights above the food prep table in the kitchen area are not shielded from breakage.

14 1. Black residue observed on the interior surfaces of the ice machine
2. Debris observed inside the soda gun nozzles
3. The can opener had an accumulation of food residue on the food contact surfaces.
20 Food items (Brussel Sprouts & Chorizo) were held at temperatures greater than 41 degrees F.
The brussel sprouts were discarded and the chorizo was placed into the walk-in freezer for a faster cool down.
41 Scrappers observed stored between the 3-basn sink and the wall, which are not easily cleaned and sanitized.
45 Loose rubber door gaskets observed on the walk-in cooler and the cold prep refrigeration units.
47 Pooled water observed at the bottom of the cold prep refrigerator
Gaskets on refrigeration units observed with dirt and food residue debris
53 Dirt/debris observed on the floor in the walk-in cooler (beverages).
53 Maintenance tools ( brooms, mops, and dustpans ) were stored directly on the floor and not elevated on a utility rack.
