14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 50 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions due to a faulty stopper and lack of tablets. **A new stoppers was purchased at the time and additional tablets were added.
20 Cheese was held at 49.8 degrees F, rather than 45 degrees F or below as required in the sandwich prep refrigerator. Deli meats and opened pre-packaged foods were held at 50 degrees F rather than 41 degrees F or below as required in the sandwich prep refrigerator. **Effected foods were removed and discarded at the time of the reinspection.
36 Observed the side door opened and not properly protecting against the entry of insects, rodents, and other animals. **The side door was closed at the time.
37 Food boxes observed on floor of walk-in freezer.
37 'Splash protection' lacks between the handwashing sink and the prep table.

13 Shelled eggs were stored above ready to eat foods in the walk-in cooler. **Shelled eggs were relocated by an employee at the time of the inspection.
13 Observed the handwashing sink in the kitchen area next to the prep sink without a splash guard in between to prevent any possible contamination.
14 Observed the 3-bay warewashing sink set up incorrectly (Sanitize, Wash, Rinse) instead of (Wash, Rinse Sanitize). **The sink was set up correctly at the time of the inspection.
26 Chlorine sanitizer solution being used in excess of 200 ppm. **Additional water was added to the solution in order to decrease the concentration of the sanitizer.
36 Visible physical evidence of insect activity (Flies) observed in the kitchen area.
