1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. * The person in charge showed up 5 min after inspector and was observed with a serve safe certificate. Inspector sent city of Philadelphia Food safety certificate via email.
8 - Paper towel dispenser empty at the 1st handwash sink in the kitchen area and in restroom area. * The person in charge provided paper towel.
8 Soap was not available at the 1st handwash sink in the kitchen area. *Soap dispenser is defective.
13 - Raw whole shell eggs observed stored above ready to eat food items in walk-in cooler. -Open canned food items observed on storage racks in walk-in cooler. *Food shall be stored in food grade containers.
36 Visible physical evidence of rodent (mouse droppings) insect activity observed in the kitchen area under storage racks to the rear of kitchen.
36 Door sweep observed missing off of front entry door (outer opening)in the food facility does not protect against the entry of insects, rodents, and other animals.
45 Fan vent cover observed missing on fan in walk-in cooler to rear of kitchen.
47 -Food debris observed on floor in walk-in cooler. -Ice accumulation observed in reach-in refrigerator.
51 -Dust accumulation observed on ceiling vent in restroom. - Door observed not self closing in restroom. Latch is broken.
53 - Food debris observed on flooring surface under equipment throughout kitchen. - Debris, dust and mouse droppings observed under storage racks in kitchen.
53 - Missing ceiling tiles observed in kitchen area. -Damaged floor tiles observed in prep area.
54 - 2 burnt out light bulbs observed under hood. - Inadequate lighting observed in walk-in cooler.
54 Employee personal items (Purse) observed in food prep area. *Purse was removed.
65 Garbage grinder observed defective.
