1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
2 No food safety certified person present to demonstrate knowledge of reportable foodborne illnesses or symptoms.
6 Proper handwashing cannot be conducted at the handwashing sink in the toilet room near the rear of the group meeting area. Water temperature at sink only reached 81 degrees rather than 100 degrees F.
14 Proper sanitization of reusable items cannot be conducted. A 3-bay sink is not available. Sanitizer and soap were placed in one sink during the inspection. The person in charge was instructed to purchase all disposable serving utensils and to remove all reusable dishes, pots and utensils from the facility.
14 Small amount of black residue was observed on the interior of the ice machine bin around the nobs/screws at the top. Please discard all of the ice and wash, rinse, sanitize and air-dry the machine prior to using it.
20 -Milk on ice for self service was held at 47 degrees F, rather than 45 degrees F or below as required. Instructed the person in charge to ensure that the milk cartons are surrounded by ice, but not submerged in ice. -Butter in the self-service area was held at 67 degrees F. Instructed the person in charge to store the butter in a plastic bag on ice. Butter should be protected from melting ice. Corrective action: Butter was discarded.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible. A probe thermometer is needed to test the temperature of food being delivered to the facility and to ensure that food is being reheated to 165 degrees F prior to service.
45 (Ref. 46.501) The following equipment is not NSF approved: -Reach-in freezer -2-bay drop-in sink -Two stove -Countertop Counter-top holding 2-bay sink is in disrepair.
47 Some debris was observed in the reach-in freezer in the prep area.
53 Brooms and dustpans observed stored directly on the floor rather than elevated.
58 Facility was first instructed to submit plans on 12-11-08; at that time it was a new operation to the Health Department. A completed Plan Review Application with proper plans, drawn to scale, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the Office of Food Protection.
